Job Description
Housing Specialist Catskill Center for Independence Oneonta, NY 13820 • Hybrid work Overview The Housing Specialist will support participants in the Olmstead Housing Subsidy (OHS) and Rapid Transition Housing (RTH) programs—specifically Medicaid beneficiaries with disabilities who are transitioning from, or have recently transitioned from, nursing facilities. This role focuses on helping individuals secure and maintain safe, stable housing by providing supportive services and managing rental subsidies. Responsibilities Conduct outreach to generate referrals for the OHS, RTH, and other CCFI services, including presentations to landlords, property managers, hospitals, managed care organizations, medical respite programs, and skilled nursing facilities. Receive and process referrals, determine eligibility, maintain a referral list, and complete intakes as appropriate. Identify available housing units within Fair Market Rent guidelines; maintain a database of local housing options including accessibility, location, service proximity, and rental costs. Provide direct support to consumers in finding and securing housing—assisting with background/credit issues, housing tours, landlord meetings, lease signings, and the moving process. Coordinate with the MFP Transition Specialist as needed. Register consumers for Section 8 and other housing subsidies; help maintain eligibility documentation. Conduct annual apartment inspections to ensure units meet program standards, and collaborate with consumers and landlords to resolve any deficiencies. Develop an Individual Service Plan (ISP) with each consumer, outlining personal goals across wellness, housing, financial, and social domains. Update ISPs every six months through in-person visits. Offer Independent Living Skills Training, advocacy, and other supports to help individuals retain housing and achieve greater independence. Skills Strong organizational skills and attention to detail. Excellent communication and interpersonal abilities. Conflict resolution and negotiation skills. Proven time management and reliability.
