Support coordination includes ongoing case management to ensure that a recipient accesses services needed to maintain health, safety, and welfare.
WSCs are responsible for assisting recipients in accessing services from all available resources and monitoring the effectiveness of home and community based services.
The support coordination service includes advocating for the recipient and identifying, developing, coordinating, and accessing supports and services on the recipients behalf, regardless of the funding source.
Support coordination can also involve assisting the recipient or family to access supports and services on their own.
Services and supports shall be obtained through a variety of funding sources, including the budget Waiver, Medicaid state plan services, third party payers, other government or community programs, and natural supports.
Nonwaiver services may include assistance through state, federal, local government, and community programs that support people at home and work.
The WSC shall facilitate the development of meaningful relationships and increased participation in the community by identifying and providing the recipient with information about community events, special interest clubs, service organizations, faith-based groups, volunteer opportunities, and competitive integrated employment that may be of interest to the recipient.
