Job Description
Service Coordinator Assistant PLAN of Massachusetts and Rhode Island, Inc. Quincy, MA 02169 Reporting to the Service Coordinator Supervisor, the Service Coordinator Assistant provides administrative support to the Service Coordinator team in service of beneficiaries and their families. Essential Job Functions Respond to requests for information related to a beneficiary’s trust account in compliance with PLAN’s policies and procedures. Conduct outreach to beneficiaries and collateral contacts to obtain written benefit verification and legal representative documentation to ensure the complete compilation of benefits, and other required documentation, for each beneficiary. Assist Service Coordinators in contacting beneficiaries, and/or their representatives, in obtaining requested information related to disbursement requests; and/or providing updates regarding the status of a disbursement. Perform annual check-ins on all Future Funded accounts; update Lifecare plans, as needed; and maintain timely and accurate notes in Salesforce documenting each encounter with Future Funded accounts. Facilitate and manage online shopping and phone orders for beneficiaries. Assist Service Coordinator team in exploring resources and obtaining the most current information related to public benefits, such as Medicaid, disability benefits, affordable housing resources and food insecurity providers; manage resource file; and maintain service coordinator manual based on latest updates from regulatory agencies and support partners. Field calls from beneficiaries and families, as needed, to support the Service Coordinator team in addressing the immediate needs of beneficiaries. Under the direction of the Service Coordinator Supervisor, assist in the scheduling of service coordinator visits with beneficiaries, as needed. Assist with beneficiary referrals to service providers/agencies for needed services. Participate in Service Coordinator meetings and maintain minutes from meetings. Facilitate the mailing of routine letters, notifications, and forms, to participants on behalf of Service Coordinators. Function as liaison to Service Coordinators in communicating potential changes in scheduled visits/appointments with beneficiaries, as needed. Document all applicable Service Coordinator Assistant activity in Salesforce in a timely manner. Assist Service Coordinator team in ensuring the accuracy of beneficiary information in Salesforce. Assist Service Coordinator Supervisor in reviewing mileage reports. Assist Service Coordinator Supervisor in completing special projects, upon request.
Qualifications:
