Role:

WeCare365 is seeking a highly organized and dependable Caregiver Support Coach to provide administrative and program support for our Adult Foster Care (AFC) program. This position plays a key role in ensuring program compliance, supporting caregivers and members, and assisting our clinical and care management teams with accurate documentation and communication.

Key Responsibilities:

Act as an administrative point of contact for assigned caregivers and members Track, document, and update caregiver/member status changes and leave requests Assist with processing and uploading documentation (ACG applications, logs, forms, etc.) Maintain accurate electronic records and case notes in the internal system Provide clear and timely communication to caregivers, members, and internal staff Monitor caregiver leave usage and ensure compliance with program limits Coordinate with multidisciplinary team (RN, CM, Compliance) regarding service changes or issues Assist with CORI checks and completion of required documentation for new caregivers Support discharge processes and changes in member care levels Answer inbound calls and respond to caregiver/member inquiries Escalate concerns or complex issues to supervisors or leadership Perform general administrative support and assist with department projects as needed

Qualifications:

Minimum 1 year of experience working with elders or individuals with disabilities Strong administrative and organizational skills Excellent verbal and written communication skills High attention to detail and ability to multitask in a fast-paced environment Proficiency in Microsoft Office (Outlook, Word, Excel, OneDrive) Ability to work independently and manage time effectively Professional, dependable, and responsive to team and client needs Must pass CORI, provide a recent physical (within 12 months), and annual TB test Knowledge of HIPAA and healthcare documentation processes is a plus WeCare 365 is an equal opportunity employer.
