The Support Services Assistant plays a vital role in helping Medicaid recipients access essential services by providing administrative and coordination support in both transportation and housing. This position involves conducting intake assessments, scheduling rides, contacting housing providers, and following up with members to ensure their needs are met in a timely and organized manner.
Key Responsibilities:
Transportation Support:
Contact members promptly when transportation referrals are assigned. Complete transportation intake assessments to gather necessary details (e.g., appointment time, location, mobility needs). Schedule rides in the system for assigned drivers with a minimum of 48 hours’ notice. Follow up with members to confirm future transportation appointments and provide updates as needed. Coordinate with internal staff and drivers to ensure reliable and timely service.
Housing Support:
Assist Medicaid recipients in identifying housing options within their financial means. Research income based housing programs and private landlords offering affordable rental units. Contact housing providers to gather up-to-date qualification criteria. Help members complete housing applications and collect required documentation. Maintain organized records of available housing resources and application statuses. Provide follow-up communication to members regarding housing opportunities.
General Duties:
Accurately document all activities, communications, and outcomes in the system. Maintain confidentiality and compliance with Medicaid and NHOO guidelines. Communicate professionally with internal teams, clients, landlords, and transportation providers. Assist with additional administrative duties as needed.
Qualifications:
- High school diploma or GED required; some college coursework in Human Services or related field preferred.
- Experience in social work, customer service, or administrative support preferred.
- Strong communication, time management, and organizational skills.
- Comfortable making phone calls and engaging with community providers.
- Proficient in Microsoft Office and scheduling systems.
- Commitment to supporting underserved populations with dignity and respect.
