Position Summary:

The Hospice Clinical Administrator is responsible for providing administrative support to the Hospice Manager with a primary focus on intake admission procedures, recertification procedures and scheduling. When needed, they work closely with all the appropriate members of the interdisciplinary team to support a highly personal and individualized plan of care. Primary Duties and Responsibilities Conduct chart reviews Entering IDG transcription attendance to all IDG meetings Initiate Admission Procedure Receive, enter, and upload patient information into the EMR. Verify signed MD orders and Certificate of Terminal Illness (COTI) Initiate client digital folder Assign and track new evaluations and orders MD correspondence as needed Track re-certifications Notify Therapists and RNs of re-certifications Upload orders Generate

day re-certifications monthly reports Support with scheduling visits and communicating with the Interdisciplinary Team (IDG). Resource to the clinical staff and the Hospice Manager regarding details of the hospice referrals to facilitate the best possible transition to hospice care. Qualifications, Skills, and Competencies:

Graduate of an accredited school of nursing for an

LPN. 1-2

years of recent patient centered care/nursing experience. Current state licensure, CPR certification, and a valid driver’s license. One year of hospice experience preferred. Strong observation, communication, problem-solving and time management skills. Physical capability to lift patients and equipment, with considerable walking, standing, reaching, bending, and kneeling.
