Service Coordinator Early Intervention Heart To Heart Children’s Services Manhattan, NY About Us:
Heart To Heart Children’s Services is a dedicated provider of early intervention services for children ages birth to three years with developmental delays or disabilities. Our team is committed to helping families access the support they need and coordinating services that promote growth, development, and positive outcomes.
Position Summary:
The Service Coordinator plays a critical role in the Early Intervention Program, serving as the primary point of contact for families. This position is responsible for ensuring that eligible children and their families receive appropriate evaluations, services, and support in a timely and coordinated manner. The Service Coordinator facilitates the development, implementation, and monitoring of Individualized Family Service Plans (IFSPs) in accordance with state and federal regulations.
Key Responsibilities:
Serve as the family’s main point of contact for Early Intervention services. Coordinate and monitor evaluations, services, and transitions. Facilitate the development and review of the Individualized Family Service Plan (IFSP). Ensure all services are delivered in accordance with the IFSP and timelines. Maintain accurate, timely documentation in accordance with state regulations. Collaborate with multidisciplinary teams including therapists, evaluators, and educators. Provide families with resources, information, and support. Monitor progress and adjust services as needed based on family input and developmental milestones. Ensure compliance with all Early Intervention Program regulations and policies.
Qualifications:
Bachelor’s degree (or higher) in human health field. Strong organizational and communication skills. Ability to work independently and as part of a team.
