Full-Time Job Summary:

The Permanency Coordinator is responsible for the completing specific administrative functions as outlined by DCS in order to facilitate legal permanency for identified children.

Accountabilities:
  • Reports to the
Permanency Support Permanency Specialist Director Essential Functions:
  • Review the prospective adoptive family’s record to establish rate negotiations.
  • Submit rate request and documentation to Central Office for approval. (All providers responsible for the child’s treatment and care will need to complete a rate justification form for consideration in the rate negotiation).
  • Complete pre-placement/presentation summary.
  • Make initial contact with adoptive family within 7 days of assignment for finalization cases
  • Request a copy of the non-recurring expenses.
  • Request the attorney to file the petition to adopt and provide the adoption date.
  • Request clearance from the Putative Father Registry within 10 days of the petition being filed.
  • Prepare the confidential court report and Consent to Adoption.
  • Submit the Adoption Court Packet to the Court finalizing the adoption within 7 days of the adoption finalization date. The court packet includes the following items: o Confidential Court Report o Guardianship Order and Surrenders o Putative Father Responses o Home Study Information o Request for New Birth Certificate
  • Other duties as required.
  • Participates in Harmony staff meetings and trainings to include, but not limited to completion of mandatory trainings to support compliance with COA standards and professional best practices.
  • Communicate with supervisor in a timely manner.
  • Prepare and submit updates as outlined.
  • Assist with weekly report calls as needed.
  • Assist staff if needed.
Job Specifications:
  • Bachelor’s degree in social services, education, humanities or related field
  • A minimum of five years of experience in the field of child welfare services.
  • Extensive knowledge of the DCS system.
  • Extensive knowledge of the administrative process of DCS adoption finalization.
  • Ability to speak comfortably with individuals over the telephone.
  • Ability to multi-task and work efficiently.
  • Ability to work efficiently independently and on a team.
  • Ability to use computer programs such as Microsoft Office, Excel and web based databases is required.
  • Part time position requires flexibility in scheduling.
  • There is no guarantee of a minimum number of weekly hours and hours may vary week to week. Maximum number of hours per week is capped at 29 in a 7-day period.
  • Ability to lift minimum of ten (10) pounds required.
  • Position is based from employee’s home.
  • Must have reliable transportation, proof of auto insurance, and a valid driver’s license.
  • Extensive automobile travel to all areas of Tennessee to meet with clients or attend adoption finalizations.
  • Must clear all Harmony required pre-employment background checks.
