In this multifaceted role, you’ll lead and empower a team of RN Case Managers while directly impacting the lives of children. What You’ll Do:
Lead, Supervise & Coordinate Comprehensive Care Supervise and support a team of 6-8 RN Case Managers , ensuring the highest level of care for medically fragile or special needs children residing in foster homes. Provide weekly supervision to your team, offering professional development opportunities, and addressing knowledge or skill gaps. Oversee caseload management , including assigning client caseloads and monitoring nurses’ ability to build and manage their assigned cases. Supervise the daily management and coordination of appointments, supplies, and medication for children on nurses’ caseloads, acting as a key resource for staff involved in patient care. Ensure timely and accurate medical documentation , including medical reports (within 2 weeks of appointments) and electronic medical records (real-time updates). Facilitate effective communication within the team (social worker, foster parent, biological parent, psychiatrist, pediatrician) regarding appointments, special assistance needed, and any follow-ups. Provide continuous feedback and support to nurse case managers, building their confidence and skill while addressing performance and resolving issues in collaboration with the Nursing Administrator. Conduct performance evaluations and manage administrative support for your team, including time and leave approvals and disciplinary actions. Ensure all staff attend required MFC education sessions . Approve appropriate equipment and supply requests for foster children. Review essential documentation developed by your team, such as school forms, reports, rate forms, and summaries. Maintain a small caseload of 4-6 developmentally disabled children , performing mandated monthly home visits jointly with case planners. You’ll conduct complete, purposeful, and detailed assessments of the home environment. Provide initial health education and skills building for foster and biological parents on their child’s medical condition, medication administration, safety precautions, and age-anticipatory guidance. Demonstrate competency in nursing practices , including proper nursing skills, use of medical equipment, and current nursing protocols. Utilize the nursing process to develop nursing assessments from comprehensive birth, developmental, health, immunization, and social histories. Assist Nurse Practitioners/Medical Doctors with well-child care, follow-up, and sick visits, and administer immunizations as directed. Triage daily phone calls and serve as an emergency on-call resource periodically, including 24-hour on-call coverage for at least 4 weeks annually. Continuously update professional knowledge through continuing education units and attend various team and program meetings. Work collaboratively with social workers, administrative staff, doctors, and community health care providers.
What You’ll Do:
Program & Administrative Oversight Provide backup to the Nursing Administrator for Program Intake , which includes screening placement requests, identifying home placement resources, introducing foster parents to children, and arranging initial health education for foster parents. Create, implement, and maintain a care plan for each identified problem at intake. Generate monthly reports of caseloads and activities for review by the Medical Director.
Schedule & Location Work Week:
Monday-Friday, 9:00 AM
- 5:00 PM, with flexibility based on connecting with children and families.
Work Model:
This role combines office and field work. You’ll work 3 days in the office (Brooklyn, NY 11201) for supervising and administrative tasks, and 2 days in the field for patient visits. You do not need to report to the office on field days and can complete paperwork from home.
On-Call:
Participate in an on-call rotation of 2 weeks per quarter , which is phone-based and typically not busy. On-call compensation includes a paid day off.
Locations:
Office is in Brooklyn. Field coverage areas include Brooklyn, Staten Island, and Manhattan.
Clinic duties are:
New York, New York 10026.
Transportation:
MetroCard provided for field visits. Rental car available if more than 3 field visits are required in a day, with mileage reimbursement. No personal car required.
Technology:
Provided with a Surface Pro, a phone, and a dedicated ZOOM account. Electronic Medical Record (EMR) system is E-Clinical Works.
Who You Are:
Credentials & Qualifications Registered Nurse (RN) New York State License. Minimum of 5 years of Acute Pediatrics experience required , with demonstrated RN supervision experience. Excellent verbal and written communication skills. Comfortable working with various Windows-based digital platforms, including Microsoft Word, Teams, and Excel. Comfortable working with and coaching others on Electronic Health Record systems (E-Clinical Works experience preferred). Goal-oriented with strong time management skills. Flexible and able to work effectively under pressure. Required to have or receive the
COVID-19
vaccine.
