This is an ideal position for a social worker, nurse (LPN or RN) or someone with prior discharge planning experience. This position is responsible for facilitating admissions from assigned hospitals on a daily basis. This position effectively gathers information and conducts on-site evaluations for our post-acute short stay program. The ideal candidate will have the ability to work independently while establishing and maintaining positive relationships with hospital Case Managers, Discharge Planners and Social Workers.

Requirements:
