Job Description
Job Coach – Job placement specialist for individuals w/ IDD Brian’s House Inc Exton, PA 19341 As part of the interdisciplinary team, helps design and implement individual habilitation plans that will lead to placement of program participants in either the least restrictive supported employment option and/or individual placement. Responsible for monitoring, documentation and programming for each individual preparing for or transitioning into placement. This position is mostly field work and reports directly to the Coordinator of Community Employment Services. Duties and Responsibilities Serves as an integral member of the multi-disciplinary team, communicating all facets of each individuals’ program to the Program Specialist. Attends all team meetings, including quarterly and annual reviews, for each individual assigned. As part of the team process, determines the supports necessary and assists in the development of a plan to provide these supports with the goal of competitive employment. Evaluates and assesses skills necessary for placement. Conducts Community Based Work Assessments with individuals referred to the program Introduces employment options to individuals by familiarizing them with different work settings. Familiarizes potential employers with benefits gained from hiring through Brian’s House Enterprises. Develops and implements task analysis-based training programs with the emphasis on having individuals perform the total job to their maximum ability. Provides on-the-job training to individuals in groups and/or on an individual basis. Makes follow-up visits to the work site for the support of both parties. Maintains positive community and customer relations. Determines the need for environmental modifications or adaptive equipment in the work setting and assists in obtaining these adaptions. Evaluate, assist and/or train needed traveling skills. Coordinates transportation arrangements, including travel training, to get individuals to and from their work sites. Required to work hours agreed upon by both the employee and Coordinator of Employment Services. Works evenings and weekends, as the needs of the individual’s work schedule dictate. Assists in maintaining necessary records and program documentation as required by Brian’s House Enterprises policies, federal/state/local laws and regulations, and licensing/accrediting bodies. Perform other duties as assigned. Qualifications Required Knowledge, Skills, and Abilities (employment) A Bachelor’s Degree in Human Services of related field is preferred but not required. A minimum of one year’s experience with individuals with developmental disabilities. Physical ability to assist as needed, which may include lifting as necessary. Knowledge of the practical aspects of training and care of individuals with intellectual developmental disabilities and mental health diagnosis. Demonstrate ability to interact in respectful, dignified manner with individuals. Ability to verbally communicate effectively with individuals, employers, business owners, staff and professional agencies in order to maintain efficient operation of agency business. Demonstrates an understanding of the differences in group supported work and competitive employment for individuals with disabilities. Must be knowledgeable with respect to various job settings and basic vocational requirements, including skills training in an industrial setting; employer expectations of integrations into the work setting; potential obstacles in the work place offsetting gainful employments; access to public transportation at possible job sites. Perform mostly field work with minimal supervision. Demonstrate writing skills to adequately perform all required documentation. Demonstrate ability/understanding of basic life safety topics relevant to individuals’ wellbeing. Ability to complete and maintain all training required by licensing or the agency. Minimum Requirements Must be 18 years of age or older, possess a high school diploma or equivalent. A car and valid driver’s license. Proof of valid automobile insurance submitted annually. Prior experience with individuals with intellectual disabilities a plus. Must be knowledgeable with respect to various job settings and basic vocational requirements, including skills, training in an industrial setting; employer expectations of integration into work setting; potential obstacles in the work place offsetting gainful employment. Must be able to lift a minimum of 50 pounds, be agile, physically pursue clients on foot, physically restrain, bend, squat, stretch, reach, push up and stand for several hours at a time. Must be able to read, write, enter dates and follow verbal/written directions, prepare written reports and complete written forms in English. Must be able to verbally communicate in English and direct clients, respond to client’s needs. Proficiency with Microsoft Office, specifically, Microsoft Word, Excel and Outlook.
