As a Community Support Coordinator , you’ll play a pivotal role in connecting, empowering, and uplifting the individual we serve by providing and carrying out the comprehensive Plan of Care. In this role, you’ll lead initiatives designed to address unique needs, foster inclusivity, and promote a sense of belonging, working closely with natural supports and an interdisciplinary team. This provides a rare opportunity to be at the heart of positive change, making a tangible difference in people’s lives by ensuring seamless plan development and implementation to enhance the lives of those we serve.

Key Responsibilities:
Training and Supervision:

Lead the training and supervision of Direct Care staff, and promptly addressing any program-related concerns reported to the Program CSC Supervisor.

Personalized Interaction:

Engage with consumers through one face-to-face meeting and two telephone contacts per week, ensuring they receive the support they need.

Independent Living Skills:

Oversee the development of essential independent living skills such as housekeeping, budgeting, accessing community resources, and more, as outlined in the Plan of Care.

Compliance and Oversight:

Maintain compliance with regulatory and funding source requirements, monitoring service delivery and providing feedback on Direct Care staff performance to the Program CSC Supervisor.

Advocacy and Support:

Advocate for consumers’ needs, assisting in securing appropriate housing, and designing behavior charts when necessary.

Financial Management:

Handle consumer expenditures when the Agency is listed as the representative payee, ensuring accurate tracking and reconciliation of bank statements.

Incident Reporting:

Act swiftly in reporting critical client incidents to relevant authorities within two hours of notification.

Individualized Planning:

Play a vital role in the preparation, implementation, and updates of individualized plans of care, attending ID team meetings and CPOC quarterly and annual planning meetings.

Progress Monitoring:

Regularly review and implement goals for CPOC and Individual Service Plans, providing constructive feedback and adjustments as needed.

Qualifications:

Bachelor’s degree (B.A. or B.S.) from a four-year college or university, with at least one year of experience in a related field. Proficiency in guiding and mentoring a team of individuals to deliver compassionate care to the individuals we serve. Strong communication skills, both written and verbal, with the ability to present information effectively to diverse audiences. Proficiency in problem-solving and the ability to interpret technical instructions. Ability to conduct meetings and handle interactions with tact and courtesy. Empathy and a keen ability to identify consumers’ needs.

Job Type:
Full-time Pay:

$19.95 – $20.08 per hour
