Position Type:
Full-Time (35 hours per week), Non-Exempt Position Summary The CCS Site Coordinator is responsible for all tiers of the Connected Community Schools (CCS) Model•Universal, Targeted and Individual supports. The CCS Site Coordinator will serve as a liaison between the district staff and CCS project management, collaborate with the school district and facilitate an assessment process that aligns with the districts strategic plan and with local and regional community resources to reduce barriers to student well-being, achievement, and school connectedness. The CCS Site Coordinator will organize and facilitate district/school level LINK and CCS meetings, and facilitate school and classroom programming and family engagement activities throughout the school district. This position also serves as the primary point of contact for the LINK operating system, responding to all incoming LINK referrals, completing individualized needs assessments, and making appropriate referrals to resources and community services.
ROLE AND RESPONSIBILITIES
Typical schedule 8am-4pm with Flexibility to work in the evening or on a weekend day Activities can include but not limited to any or all of the following depending on the need of the connected districts: Become familiar with the nature of a Community School and its Pillars. Become familiar with the Connected Community Schools Model. Become familiar with Connected Community Schools policies and procedures. Works with school district staff, including but not limited to district and building administration, guidance, and social work staff, to learn district wide policies and procedures, as well as building specific policies and procedures. Attends school meetings as needed. Become familiar with existing school resources and programs. Attends and completes all trainings and certifications as required by the Connected Community Schools Program. Work with the CCS team to conduct the district Needs Assessment and school climate survey. Responsible for Resource Mining in the districts local community; shadowing programs, outreach to organizations, businesses, agencies in community in order to develop relationships and partnerships that can be accessed for the Community School. Assist in development, monitoring and overall coordination of Youth and Family Resource HUB. Organization and maintaining inventory of the tangible resources within the hub (I.e. food, clothing, school supplies, hygiene products etc.). This will include physical lifting of the tangible items within the resource hub. Organization and/or facilitation of Instruction to the students and/or adults within the district (I.e. Adult Learning Programs, Life Skills etc.). Oversight of the school District Food Bank (applicable only to schools with an embedded, certified food bank). Attend appropriate CNY Food Bank Trainings Responsible to track the data collection and reporting, in collaboration with the Project Manager and/ or the Operations Manager In coordination with the Project Manager and/ or the Operations Manager, order items needed for the Food Pantry Ensure all food bank guidelines are adhered to at all times at the food bank site location. Outreach and engagement with the students, staff and or families that are accessing the hub. Attend CCS Advisory Board Meetings and all other appropriate community-based meetings as assigned. Responsible for the development and coordination of the District/building LINK Team meetings. Implement and facilitate ongoing Site Committees to review and discuss all CCS Activities; Utilize existing Community Schools outline structure of a Site Committee. Facilitate minimally 1 program each month; utilizing the data from the Needs Assessment to focus on the identified goals of the school district Facilitate minimally 2-3 classroom activities per month Recording and submitting meeting minutes as directed. Perform LINK outreach and engagement for all referrals assigned, including but not limited to: Screening/assessment of needs for identified children and families through utilization of the SCAN. (Screening for Comprehensive Assessment of Need) Tool. Completing applicable referrals based on need. Providing outreach and engagement to enroll referred individuals into the Health Home program or work collaboratively with the HHSC Care Management team to facilitate this process. Collaborating with providers, family, and school faculty to establish appropriate support networks as needed. Follow-up communication with school referent regarding outcome of referral and with student and family to ensure successful service connection. Collect, track, and maintain LINK data with the utilization of project-specific trackers, SchoolTool, and other applicable resources and produces appropriate weekly and monthly outcome data as directed by the Operations Manager. Traveling between connected district sites, administrative office and/or community locations as designated by the Connected Community Schools Management Team for in person meetings and/or trainings. District/Community Wide Programming events (I.e. Family Engagement Night, Community Wide Food Drop etc.) Additional duties assigned as applicable to the needs of the district and/or community school initiative Universal Job Functions Establish a work environment that creates positive communication between supervisors and employees and assimilates new employees to the agency culture, values, and mission. Ensure compliance with all pertinent government and agency regulations and operating standards, including maintaining all required documentation and applicable databases. Assist with special projects as needed. All other duties as assigned.
QUALIFICATIONS / EDUCATION / EXPERIENCE REQUIREMENTS
Minimum of a High School Diploma with 5-10 years’ experience in a Human Service field. An Associate’s Degree or Bachelor’s in Human Services Preferred (salary based on education and experience). Basic Computer Skills (Windows, Outlook, Word, Excel) Travel is required. Must have a valid NYS Driver’s License. III PHYSICAL DEMANDS
Certain deadlines and unanticipated developments may require work during evenings, weekends. Ability to quickly address any emergent issues without losing focus on task at hand. The employee must have full sight and hearing with fluency in the English language. While performing the duties of this job, the employee is regularly required to talk or hear. The employee frequently is required to stand, walk and sit. Must be able to sit for long periods of time. Constantly operates a computer and other office productivity machinery, such as a copy machine, scanner, computer printer, etc. The employee will need to occasionally lift and/or move up to 25 pounds. The physical demands described here are representative of those that must be met by an employee to successfully perform the essential functions of this job.
