The ideal candidate will help build meaningful connections with individuals and families by offering guidance and access to supportive care resources.

Job Function:

Day Shift. 9am – 5pm/ 10am – 6pm. Link residents to services that promote recovery, independence, and self-determination. Facilitate group meetings focused on Wellness Management and recovery strategies. Complete service plan reviews for all residents in a timely and thorough manner. Conduct regular inspections of apartments and personal living spaces, reporting and addressing any issues as assigned. Provide case management services, support residents in setting rehabilitation and housing goals, and collaborate on developing individualized Housing Support Plans when needed.

Job Requirements:

Peer Certification is preferred. 2 years of active participation in mental health self-help activities, peer support or advocacy programs, participant-run organizations, or similar programs is required. Demonstrated understanding of community resources relevant to mental health services.
