Through our Street to Home outreach program and transitional housing resources, we help people get, and stay, on the path to a permanent home. Assistant Director, Models of Care Reporting to the Senior Director, Models of Care, the Assistant Director, Models of Care collaborates with the Senior Director, Models of Care to develop, implement, and refine service delivery models in street outreach, transitional housing, and permanent supportive and affordable housing portfolios through training, coaching, providing technical assistance, and integrating program participant feedback. The Assistant Director, Models of Care provides direct technical assistance and training support to a portfolio of programs to promote and support the implementation of high-quality, client-centered service delivery across a large and growing Programs Department.
ESSENTIAL DUTIES AND RESPONSIBILITIES
+ Coordinate, and/or facilitate training, coaching, and technical assistance to support the implementation of the “Breaking Ground Practice Profile” + Develop and maintain department training calendar + Develop and maintain catalog of external training availabilities and sources + Collaborate with Programs Administration team to keep departmental training plans up to date in Learning Management System (Relias) + Serve as a point of contact for departmental systems and initiatives that enhance service delivery (e.g., Health Information Exchanges such as Bronx RHIO, PSYCKES, and Opioid Overdose Prevention Program certification) + Collaborate with Senior Director, Models of Care, to develop quality improvement plans with departmental leadership to support ongoing implementation of “Breaking Ground Practice Profile” + Support development and implementation of project plans and business process improvements + Liaise with department leadership and Breaking Ground Medical Director and Assistant Medical Directors to support development and implementation of best practices in care coordination and facilitate meetings for staff who liaise with onsite medical partners + In coordination with Senior Director, Models of Care, facilitate quarterly incident review/incident response in-service at programs within portfolio + Perform other duties as assigned
MINIMUM REQUIREMENTS
+ Graduate-level study in public health, public administration, public policy, or social work preferred + 2 years’ supervisory experience strongly preferred, including a minimum 1 year experience in homeless services or related field + Certification required: Naloxone dispensing. Must obtain within first 3 months of employment + Understanding of relevant service delivery concepts and structures, including Housing First, Motivational Interviewing, harm reduction models, and behavioral and medical health systems + Demonstrated training development and facilitation skills + Creative, strong problem-solving ability + Ability to delegate and motivate staff and program participants + Ability to communicate (verbally and written) with diverse populations and stakeholders + Proficiency with Microsoft Office Suite programs including Word, Excel, Outlook, and Microsoft Teams. + Familiarity with common human services software applications preferred + Bilingual preferred EOE/M/F/Vet/Disabled Job Details Pay Type Salary Hiring Min Rate 75,000 USD Hiring Max Rate 82,000 USD
