Case Manager for an Emergency 90-Day Transgender Housing Facility in Bushwick 3.0 3.0 out of 5 stars 1078 Bushwick Avenue, Brooklyn, NY 11221 Destination Tomorrow is seeking a motivated service-oriented LGBTQ+ friendly person to be our Housing Case Manager at our SWITCH Housing Program which is a 90-day emergency housing program for Transgender, gender non-conforming, and gender non-binary individuals who are victims of domestic and sexual violence, victims of sex trafficking crimes, or are participating/have participated in sex work. Duties and Responsibilities Develop and maintain a comprehensive network of education, training, mental health, substance abuse, economic development, and other supportive services for adults who have been chronically homeless Build effective relationships with residents through regular contact in person, by telephone and in writing. Proactively develop and promote self-reliance by helping residents access community resources. Complete well organized case files that accurately reflect services provided and compile monthly reports and case notes accurately and on a timely basis. Follow up on all referrals to ensure that adequate, appropriate services are being provided. Coordinate supportive services (i.e. mental health, transportation, substance abuse, etc.) with appropriate community agencies. Other duties as assigned
