56 / hr Jun 16, 2025 Join a dynamic team who provide care for our 140 + bed facility, caring for long-term residents as well as short term post-acute care patients who have been recently hospitalized.
This provides a broad spectrum of clinical experience, with continuity of care that includes both chronic and acute illnesses in adults of all ages. We collaborate with an interdisciplinary team of doctors, nurses, physical therapists and care managers to provide excellent care in a rewarding environment. Positionis a 0.5 FTE. This includes with the potential for flexible hours. Day-to-day responsibilities include admitting and discharging patients, routine periodic visits, addressing acute issues as they arise, and facilitating specialty and end-of-life care as appropriate. Requires ability to obtain DEA certificate and VT Nurse Practitioner or Physician Assistant license.
