OVERVIEW
Duffy Health Center is nonprofit, Federally Qualified Health Center (FQHC) located in Hyannis, MA. Duffy Health Center primarily serves patients who are homeless or at risk and provides primary care, mental health, substance use disorder treatment, and case management services. JOB OVERVIEW- This position will require a writing sample. The Grants Manager is responsible for writing grant proposals in support of Duffy Health Center’s mission, vision, and strategic plan, with a focus on programs and services for individuals experiencing homelessness in Barnstable County (Cape Cod). Duffy Health Center is a patient-centered medical home, and every role requires functioning effectively using a team-based approach. This role includes research on funding opportunities, writing grant proposals, stewarding funder relationships, and reporting to funders on grants outcomes. This position is deemed non-essential
- in the DHC Emergency Operations Plan.
KEY RESPONSIBILITIES 1.
Grant Research, Writing, and Application Submission a. Identifies funding gaps and needs within Duffy Health Center’s scope of service through regular outreach and meetings with CFO, program directors, and others. b. Researches, develops, writes, and submits on-time, accurate, compelling grant applications in response to public and private grant opportunities in the areas of homeless health care, outreach, mental health and substance use disorders, spiritual care, case management and recovery support services. Coordinates application preparation with program directors. c. Actively monitors federal, state and local funding opportunities. Evaluates grant announcements to determine eligibility and funding probability. 2. Grant Awarding and Agreement a. Reviews all documents when grants are received, including: the terms and conditions for utilizing the funds; reporting obligations; project timelines; deliverables. b. Ensures all appropriate stakeholders and program directors are informed of the terms, requirements, and deadlines associated with each grant. c. Ensures grant agreement is signed and submitted on-time. 3. Monitoring and Evaluation a. Monitors spend-down of grant so that funds are spent appropriately and in the allotted time period via monthly program director meetings and regular communication b. Ensures compliance with grant regulations c. Ensure grant narratives, budgets, and deliverables are aligned with FQHC guidelines and regulatory requirements . d. Coordinate with compliance and program staff to ensure adherence to HRSA and other federal guidelines. e. Creates and submits, in coordination with appropriate internal stakeholders and program directors, any interim reporting required by the grant funder 4. Grant Closeout and Reporting a. In coordination with program directors, creates final documentation required by the funder which may include reporting, narrative, final budget, and other 5. Funder Stewardship a. Nurtures and cultivates strong relationships through collaborations with community agencies who are partners and/or supporters of Duffy programs. b. Contributes to the development of academic, community, state and national health and human services partnerships. 6. Other Responsibilities a. Runs monthly overall grant review meeting with program managers to monitor spending and progress on grant goals. b. Understands and monitors trends in health care delivery as relevant to the role in the community health center and health care for the homeless setting. c. May hire/supervise consultants and/or interns for research and other special projects relevant to the role. d. Provides timely reports to management and board. Reports on annual goals for the department and self.
QUALIFICATIONS & SKILLS
Education & Experience:
- Bachelor’s degree
- Minimum three years of experience in government grant writing (state and/or federal)
- Experience in community health center or medical environment preferred
- Exceptional writing, spelling, and grammar skills
- Ability to translate metrics into compelling stories
- Strong communication skills and desire to work collaboratively
- Strong MS-Office skills
- Experience with online research
- Experience with CRM and/or grant management system
Work Environment:
Hybrid with days on-site in an office environment on Cape Cod within a community health center setting and remote. Regular interaction with both clinical and administrative staff.
Job Type:
Full-time Pay:
$63,000.00 – $83,000.00 per year
