Community Support Specialist WeCare 365-Adult Foster Care/Adult Family Care 528 Pleasant Street, Brockton, MA 02301

WeCare 365 AFC provides in-home nursing oversight and care management with a commitment to quality care for our members, caregivers, and families. Our team enjoys a supportive work environment with helpful staff and in-office resources for remote employees.
Position:
Community Support Specialist Reports To:
Manager of Care Management Key Responsibilities:
Conduct on-site visits with members in qualified settings, including initial intake assessments. Complete detailed progress notes for each visit and member encounter. Provide culturally appropriate health education, support, and counseling to members and caregivers. Act as a liaison between clients/families and community-based support services. Collaborate with the interdisciplinary team to ensure adherence to all Adult Foster Care regulations and company policies.
Qualifications:
Spanish and Cape Verdean speakers are strongly encouraged to apply. High school diploma or GED required. Strong leadership, communication, and organizational skills. Commitment to a multidisciplinary approach and quality care. Up-to-date CORI, and annual TB test.
Work Environment:
Frequent travel within a defined area in Massachusetts. In-home visits with potential exposure to clinical equipment, medications, and cleaning materials.
