POSITION SUMMARY

The Care Management R.N. Supervisor works as part of a diverse team in a multicultural Community Health Center. This supervisor will assist the nursing team with complex coordination work; offer regular clinical and programmatic support and oversight to the nursing team; collaborate and work effectively with internal departments; facilitate communication among all providers (across all disciplines represented at BNHC); empower the team to help patients to become active participants in their healthcare. The RN Supervisor will also maintain a small case load of patients living with complex and/ or chronic conditions. This position requires flexibility and may vary from day-to-day to meet members where they are. Outreach methods may vary based on the needs of the organization and may include telephonic or in person in a variety of potential settings such as but not limited to, the health center, community, home or an inpatient facility.

PRINCIPAL DUTIES AND RESPONSIBILITIES

Works under the guidance of the Program Leaders (Manager and Director). Responsible for ongoing mentorship such as guiding, reviewing, and approving the work rendered by the R.N. Care Managers. Supervise R.N. Care Managers in outreach, engagement and retention activities to support the community-based, health and wellness initiatives. Leads the R.N. Care Managers in regular individual and team meetings to support the ongoing professional development and reviews weekly performance measures. Foster team building activities and ongoing team work. Provide training to the R.N. Care Managers on health center and grant program EMR. Perform regular chart audit reviews to ensure compliance with documentation requirements and deadlines and provide timely feedback to the team for adjustments to be made. Attend trainings and meetings as necessary and appropriate. Collaborate with Program Manager to facilitate meetings with community partner agencies. Build partnerships with community organizations, service providers, and other entities to work collaboratively in support of BNHC patients. Serving alongside the Manager by ensuring that all grant fiscal, administrative and reporting requirements are met through relevant recording keeping and data gathering. Responsible for participating in hiring, training, supervising, performance evaluation, disciplining, and guiding the professional development of the R.N. Care Manager staff. Will care manage a reduced case load of patients (.5 FTE) while also performing supervisory duties (.5 FTE), adhering to established protocols and procedures. Travel as necessary for meetings and trainings. Other duties as assigned by Program Manager.

Demonstrated success in working as part of a multi-disciplinary team including communicating and working with Providers, Social Workers, Community Health Workers and other health care teams. Ability to flexibly utilize clinical expertise to solve complex problems Bi-lingual Experience working with patients with chronic medical and behavioral health needs Must be flexible and adaptable to change. Qualifications:

Experience working with a Medicaid/Medicare population is strongly preferred Experience working with Federally Qualified Health Centers is strongly preferred Associate degree in Nursing; Bachelor’s Degree in Nursing preferred Current, active MA Registered Nurse license Case Management Certification (CCM, ANCC RN-BC) preferred 3-5 years of nursing experience, preferably in-home health, ambulatory care, community public health, case management, coordinating care across multiple settings and with multiple providers A valid driver’s license and provision of a working vehicle Experience with anti-racism activities, and/or lived experience with racism is highly preferred
