Purpose:

The Street Outreach Coordinator is responsible for managing and coordinating outreach and programming for youth experiencing homelessness or at risk. This includes fostering community partnerships, enhancing program effectiveness, and ensuring compliance with organizational and grant requirements.

Qualifications:
Education:

Bachelor’s degree in Social Work, Psychology, Human Services, or a related field; Master’s degree preferred.

Experience:

Minimum of 3 years in street outreach, case management, or youth services, with demonstrated leadership and program coordination skills.

Knowledge:

Strong understanding of homeless youth issues, crisis intervention, and trauma-informed care principles.

Technical Skills:

Experience with HMIS data entry and reporting; familiarity with grant compliance and reporting.

Communication:

Excellent verbal and written communication skills, with the ability to engage effectively with diverse stakeholders.

Autonomy:

Proven ability to work independently, prioritize tasks, and manage multiple projects in a fast-paced environment.

Mobility:

Valid driver’s license and reliable transportation; ability to travel regularly within the community.

Commitment:

Key Responsibilities:

Program Coordination and Compliance Oversee daily operations of Street Outreach and Core Collective Hub Programming, ensuring alignment with organizational goals and grant requirements. Review daily logs and client progress notes for quality assurance and accurate documentation. Monitor program compliance and adherence to grant and partnership requirements. Compile and analyze statistics to support grant proposals, reports, and program evaluations. Crisis Intervention and Case Management Provide crisis intervention and case management services, including tangible resources and referrals, for youth experiencing homelessness or at risk. Conduct TAY-VI-SPDAT and other housing assessments to determine eligibility and needs. Enter and manage data in the HMIS system, adhering to all guidelines. Community Engagement and Training Increase community education and awareness of Core Collective services through strategic outreach and partnerships.  Provide training and educational presentations for youth, adults, and service providers on relevant topics.  Participate in community collaborations, service provider networks, resource fairs, and networking opportunities. Team Management and Development Develop strategies for optimal team performance and resource utilization. Manage and dispatch Street Outreach and Program team members, ensuring effective service delivery. Supervise SOP Specialists and Youth Development Specialists (YDS), providing guidance and support. Program Evaluation and Improvement Consistently review and assess program effectiveness, implementing improvements based on data-driven insights and client feedback. Collaborate with the Housing Director to develop and implement Hub out-of-school time programming. Additional Responsibilities Participate in the on-call rotation for 24-hour response to TLP and SOP needs. Attend community and service provider meetings/events to strengthen collaborations. Coordinate presentations and represent the organization at various events. Perform other duties as assigned to support programmatic goals and organizational priorities.

Job Type:
Full-time Pay:

$39,000.00 – $43,500.00 per year
