About the Role Join Callen-Lorde and play a pivotal role in increasing access to integrated care! As the Evaluator, you’ll be designing and leading all evaluation efforts. This includes direct patient data collection and close collaboration with our clinical teams, all to ensure top-notch performance, continuous quality improvement, and full compliance with grant requirements. Key Responsibilities Grant Management & Evaluation + Develop, implement, and monitor a comprehensive evaluation plan addressing program goals and objectives, performance measures, program evaluation questions, and data collection, analysis, and reporting methods. + Collect relevant SAMHSA NOMS and other data through electronic means, via providers, and through direct meetings with patients by phone, in-person, and via telehealth platforms. + Produce and disseminate data and evaluation summaries to key program staff, senior management, and SAMHSA grant officers; provide ongoing evaluative feedback to program staff and implement quality improvement strategies to improve outcome measures. + Assist with the timely submission of required data reports to SAMHSA. + Collaborate with the Finance Department to monitor grant budgets; regularly track grant expenditures against approved budgets; initiate budget modification requests as needed. Data Systems & Policy Development + Develop policies, protocols, and procedures related to data collection and data management to ensure program data are gathered efficiently and accurately. + Implement trainings to ensure program staff are effectively and consistently adhering to data policies, protocols, and procedures. + Collaborate with Callen-Lorde’s HIT team to ensure the agency’s EHR system can appropriately capture and report on program data and performance/quality measures. + Work closely with HIT, IT, and internal and external EHR specialists to review grant-related documentation procedures (including use of tablets), explore IT/HIT needs of providers, and work with management on a plan for increasing efficiency of the EHR and IT network for documentation of substance use. + Develop Callen-Lorde’s policies and procedures related to substance use screening, assessment, treatment, and referrals. + Oversee the collection of data related to substance use screening and care at Callen-Lorde; work with Quality Management teams to set and meet benchmarks for substance use identification and care. Inter-departmental Collaboration & Outreach + Collaborate with DCOs and referral partners to facilitate the development of data-sharing and data-use agreements. + Assist in workflow redesign, facilitating the process of screenings and referrals in and from other departments, and ensuring patients receive proper follow-up care. + Work with an interdisciplinary team to assess agency training needs, specifically in substance use screening and the provision of general care. + Develop outreach plans to patients who are disengaged in care to ensure that patients living with addictions have needs met, with a specific focus on LGBTQIA

  • identified patients.

+ Provide grant-related trainings to multidisciplinary teams, including but not limited to medical providers, behavioral health providers, nurses, care coordinators/case managers, and dental providers. + Develop external partnerships, creating seamless systems of information sharing and referrals to and from other facilities. + Work with the behavioral health team to create more robust programming for substance use in existing clinics with a special focus on opioid and crystal methamphetamine use. Qualifications + Licensure and certification in New York State as an LMSW, LCSW, Clinical Psychologist, or Psychiatrist + Expertise in substance abuse; experience working with LGBTQ, homeless and PLWHA communities + Experience implementing community-based quantitative and qualitative research and evaluation processes, and/or SAMHSA or other federal grants experience preferred + Demonstrated proficiency in project management, quality improvement, and data reporting and budgeting + Commitment to diversity, equity, and inclusion work, specifically with a focus on creating an anti-racist and anti oppressive culture and work environment Desired Characteristics + Demonstrated commitment to providing sensitive, culturally competent psychiatric care to LGBTQ persons and persons living with HIV, especially disenfranchised persons and persons of color + Demonstrated commitment to collaborative, team based substance use treatment valuing the input and perspective of the full array of health professionals + Ability to manage multiple responsibilities and participate in multiple clinical teams + Demonstrated ability working with trainees and educating multidisciplinary staff is required + Commitment to community based health and substance use services
