Full Job Description Position Overview:

The Community School Case Manager Supervisor in collaboration with the Director of Truancy Services, is responsible for spearheading GPCA’s mission, values, and commitment through their team. In effort, to increase organizational stability, break the cycle of intergenerational poverty, and have a greater impact on generational poverty by working with students and families in an education setting. The CSCM Supervisor is a pivotal role, responsible for overseeing a team of Community School Case Managers in encompassing both prevention and intervention efforts to support the students and families of the Community School. This position involves providing clinical supervision to team members to promote the delivery of consistent, supportive, and effective quality services. The CSCM Supervisor will be required to have on-site work hours at assigned school and GPCA site; to actively engage in developing and maintaining partnerships within the school, student and families, and establish collaborative relationships within GPCA to improve student attendance.

Key Responsibilities:

Model and coach Case Management team on GPCA’s mission, values, and commitment. Adhere and model agency Policies & Employee Handbook, Community School Program Standards, and professional code of ethics. Ensure Community School Case Managers are polished and adherent to funder and organization policies, procedures, and regulations. Assist w/ recruiting new hire within 30 days of vacancy. Assist with onboarding and training new hires within 30 days of start date Oversee a team of 3 Case Managers. The CSCM supervisor conducts case conferences with each Case Manager on a bi-weekly basis(minimum) to determine progress with service objectives, goals, and ensure compliance with TIPS standards. Provide ongoing coaching for staff in areas of best practices for client work, documentation, and professionalism. Create and support documented plans of improvement when challenges arise. Actively engage and maintain partnership with school team to coordinate on programmatic stability and identify areas of development. Provide case coverage as needed according to contract capacity or in the event of staffing shortages/ staff absence. Collaborate with the Administrative Coordinator to assign new referrals to Case Managers within 24 hours of their receipt. Monitor and ensure all program service engagement/delivery standards are being met in accordance to Community School Case Management Program Standards or as required by agency Facilitate monthly team meetings, providing positive direction to encourage the program’s quality performance Manage staff files through monthly file reviews, track 20 hrs. of training completion, and complete annual staff reviews. Attend monthly funder meetings and complete quarterly funder reports. Act as a point of contact for the funder, School District personnel, and other relevant partners for all required information as requested Ongoing collaboration with GPCA’s PQI Department on implementing performance and quality improvement measures Review budgets monthly with the Director of Truancy Services Participate in annual budgeting and contracting processes Ongoing collaboration with Truancy Director and Program Quality Improvement staff on other quality and compliance related goals as needed

Requirements:

Masters in Social Work or in related field 2+ years of supervisory experience Basic accounting and data management skills Proficiency in Microsoft Suite (Excel, Outlook, Access, etc.) Organize, consistent, and detail-oriented Exceptional written and oral communication skills Health Assessment w/ TB test Child Abuse, Criminal, and FBI clearances (performed by GPCA after offer made and signed) Mandated Reporter training (if not trained in it, will undergo it post-hire) 40+ hours per week, 9 AM to 5 PM availability (some flexibility may be required) Spanish/English bilingual preferred, but not required To perform this job successfully, an individual must be able to perform each essential duty satisfactorily.
