This position is perfect for an experienced healthcare professional. Successful candidates will have a passion for human kindness and wellness for life. This position may offer some flexibility, but it is primarily a site-based position and requires coming into the office. Does NOT participate in On-Call rotation.

Responsibilities include:

Complete

ICF & PPS

MDS’ in a timely manner RAPS Comprehensive Care Plans on ICF & Skilled Residents Update care plans with quarterly’s 30 days and 60 day Medicare assessments Provide care plan schedule to Social Worker in timely manner Schedule weekly assessments with Recreation Director, Social Worker, Dietician & Rehab Director Complete overall Quarterly and Annual schedule of MDS assessments for ICF residents Complete monthly schedule of ICF MDS assessments due for Unit managers on 1st floor 2nd and 3rd floor Able to complete assessments utilizing MDS 3.0 according to long term care regulations Proficient with computer skills MDS 3.0 experience Knowledge of Point Click Care systems Scheduling interviews now to come meet our team, tour our community, and see what HumanKindness means at
