Full Time – Intake and Transitional Housing Coordinator
SUPERVISOR
Residential Programs Director HOURS:

CLASSIFICATION
Exempt, At-Will Position The Intake and Transitional Housing Coordinator (ITHC) is a member of the Residential Program Staff that manages incoming referrals for all Palmetto Place programs, conducting assessments and coordinating intake for youth ages 15-24 that are experiencing or at-risk of experiencing homelessness, or are in the DSS foster care system. Additionally, the coordinator manages Case Management for our Transitional Housing Program, providing comprehensive planning and support for 6-8 young adults ages 18-24 in our 6-month residential program. The ITHC collaborates with the Residential Staff Supervisor and Direct Care Staff to support youth in their transition toward independent living. Referrals and Intake (40%) Receives and reviews referrals, providing direct follow-up within 24-business hours of receipt Conducts intake interviews and assessments, determining program eligibility and appropriateness for current services Coordinates with Residential Case Coordinators and Staff to conduct safe and efficient client intake Provides Resource Navigation for referrals that do not meet eligibility requirements for current programs Maintains systems for the Referral Line, Website Referrals, and Referral Spreadsheet to ensure tracking and follow-up with all received referrals Transitional Living Program (40%) Oversee management of the Transitional Housing Program, ensuring fidelity to grant standards as established by the Department of Housing and Urban Development (HUD) Provide wrap-around Case Management to six youth ages 18-24 in our Transitional Housing campus Collaborate with youth to develop individualized care plans that support their transition to an independent living setting, including career development, education, mental health and medical care, and community support Provide transportation support to youth for work, school, and/or appointments, as needed Collaborate with staff to handle challenging situations, critical incidents, or emergencies Participate in a weekly Case Management meeting to review pending referrals, discuss Client concerns, and assess Client progress in a collaborative format Data and Information (20%) Input documentation and provide reports for Aftercare Client data within digital platforms including Apricot, and Excel Utilize analysis of Client Progress, Goals, and Outcomes to provide informed decisions on program efficacy and organizational strategic planning Manage and analyze Referrals, providing reports on trends to assess community needs, organizational capacity, and program efficacy Additional Duties Participate in the on-call rotation for Safe Place coverage on weekends (Friday 11 PM – Sunday 11 PM) Completion of a minimum of 16 hours of training per year Perform other duties as assigned to meet organizational needs
Schedule:
This is a full-time role requiring a minimum of 40 hours per week, primarily on-site during business hours of 9am-5pm. Flexibility is essential to meet organizational and Client needs – evening and weekend hours may be necessary to meet agency needs. This position serves primarily at our Columbia campus.
Qualifications:
Educational requirements: bachelor’s degree in social work or related plus 2 years of experience working with at-risk population ages 18-24 Strong Organizational and communication skills, including document management, time management, and compliance with DSS licensing standards Proficiency in Microsoft Office, including Excel, Word and Outlook Excellent public speaking skills and ability to engage and build community partnerships Relationship Management skills- building and maintaining rapport with clients, peers, and community partners Ability to work autonomously and collaboratively in a team setting while working with various personalities to ensure the best intentions are held for the residents, staff, and organization Dedication to the mission of Palmetto Place and a passion for supporting youth (ages 15-24) while serving as a role model Ability to maintain flexible schedule to meet the needs of the organization

Valid driver’s license with a clean driving record Satisfactory reference checks Pass background checks (SLED, Fingerprint, DSS Central Registry, and both SC and National Sex Offender Registry) If lived outside of South Carolina in the past 5 years, must also pass DSS Central Registry check from the state previously lived Complete and pass a drug screen and Physical CPR and First Aid certification Must be 21 years of age or older Completion of Trauma-Informed Care training within first month of employment
