Full Time – Transitional Services Coordinator

SUPERVISOR

CLASSIFICATION

Exempt, At-Will Position The Transitional Services Coordinator (TSC) is a member of the Community Programs staff that manages the comprehensive Aftercare Case Management for youth ages 15-24 that are exiting Palmetto Place’s Residential Programs. The TSC will coordinate with Residential Case Coordinators to create an individualized transition plan and support each youth in successfully transitioning out of Residential Programming through independent living or family/fictive kin reunification. After their exit from the program, the TSC will engage in 12-months of wrap-around case management for each Aftercare youth, providing resource navigation, goal setting, community engagement, and independent living skill building, reducing the youth’s risks of returning to homelessness. This will require travel throughout the Richland and Lexington county to meet with youth in their community. The Transitional Services Coordinator will also oversee the scheduling of Independent Living Skills sessions for all Programs, based on the curriculum and content outlined in the Casey Life Skills Assessment. The TSC will facilitate at least 5 Independent Living Skill sessions per month and coordinate with Direct Care Staff and community partners to schedule and facilitate Skill sessions in the areas of Financial Literacy, Career and Education, Basic Life Skills, Health and Wellbeing, and Community Connections.

Primary Responsibilities:

Aftercare Services (40%) Coordinate with Residential Program staff to plan and facilitate successful transition plans for youth exiting Residential Programs Provide 12-months of wrap-around Case Management for all youth transitioning out of Residential Programs Collaborate with the Department of Social Services to assist eligible youth in utilizing their Chaffee, ETV, and Independent Living benefits Ensuring fidelity to grant standards in Aftercare for the Basic Center Program, as established by the Family and Youth Services Bureau Co-facilitate monthly events for Community Youth to promote independent living, skill building, and foster community relationships Participate in a weekly Case Management meeting to review pending referrals, discuss Client concerns, and assess Client progress in a collaborative format Create content for the Youth Instagram, sharing resources, community events, and providing an engaging platform for community youth to connect Independent Living Skills (30%) Utilize the Casey Life Skills assessment to maintain a quarterly schedule of Life Skill sessions for Residential and Community Programs Facilitate a minimum of 5 Life Skill Sessions per month for Residential and Community Programs Collaborate with Palmetto Place Staff and Community Partners, providing instructional guidance on the facilitation of Independent Living Skills Create Life Skill lesson plans that include educational content, applicable worksheets, and instructional guides Data and Information (30%) Input documentation and provide reports for Aftercare Client data within digital platforms including Apricot, and Excel Utilize analysis of Client Progress, Goals, and Outcomes to provide informed decisions on program efficacy and organizational strategic planning Additional Duties Participate in the on-call rotation for Safe Place coverage on weekends (Friday 11 PM – Sunday 11 PM) Participate in Community Outreach to build partnerships that support the mission of the organization Completion of a minimum of 16 hours of training per year while ensuring all staff meet the same requirements Perform other duties as assigned to meet organizational needs

Schedule:

This is a full-time role requiring a minimum of 40 hours per week, primarily during business hours (9 AM – 5 PM). Flexibility is essential to meet organizational and Client needs. Time is split between the two Midlands campuses, as well as across Richland and Lexington County to meet with clients in the community. This is not a remote position.

Qualifications:

Educational requirements: bachelor’s degree in social work or related plus 2 years of experience working with at-risk population ages 18-24 Strong Organizational and communication skills, including document management, time management, and compliance with DSS licensing standards Proficiency in Microsoft Office, including Excel, Word and Outlook Excellent public speaking skills and ability to engage and build community partnerships Ability to work autonomously and collaboratively in a team setting while working with various personalities to ensure the best intentions are held for the residents, staff, and organization Dedication to the mission of Palmetto Place and a passion for supporting youth (ages 15-24) while serving as a role model Ability to maintain flexible schedule to meet the needs of the organization

Pre-Employment Requirements:

Valid driver’s license with a clean driving record Satisfactory reference checks Pass background checks (SLED, Fingerprint, DSS Central Registry, and both SC and National Sex Offender Registry (If lived outside of South Carolina in the past 5 years, must also pass DSS Central Registry check from the state previously lived) Complete and pass a drug screen and Physical CPR and First Aid certification Must be 21 years of age or older Completion of Trauma-Informed Care training within first month of employment
