Job Summary:

This position works under the direction of the Executive Director of Care Coordination and supports the functions of Care Coordination as well as day to day operations of the department. This position is responsible for generating detailed, customized, analytical reports by performing accurate calculations and creating spreadsheets and databases that will drive clinical performance utilizing data for medical management to support WellStar hospitals. Proficient in utilizing multiple databases and independently create complex reports to support the proactive identification of patient populations and financial risk. Direct responsibility for meeting the information needs of our internal and external customers by identifying trends, both negative and positive which may indicate the need for change and action plans. Performance includes knowledge of performance indicators within the department such as coordinating daily reports on active patient populations to assist in securing timely authorizations for hospital days and identifying patients with extended length of stays. Daily reports are utilized by all of WellStar’s acute care hospitals. Supports team members/committees with detailed presentations and worksheets, work on projects for the department and in the clinical environment and must have the ability to support multiple functions concurrently. Ability to quickly learn new tasks, data tools, and structure with minimal assistance in order to produce reports is a must.

Core Responsibilities and Essential Functions:

Coordinate functions of the department using effective project management.

  • Assist Care Coordination department with hospital wide committee duties facilitating Medical Management. Assist with coordinating meetings, minutes and other communications to team.
  • Create and set up computer generated reports to support the functions of both Medical Management and care coordination daily functions
  • Data analysis and reporting of disease specific populations.
  • Assist with developing graphics and presentations for hospital committees.
  • Proactive management in updating and keeping presentations current with new data and identified trends.
  • Assist with preparing presentations for and onsite at meetings including development of reports and slides
  • Assist with integration of new departments and initiatives into Care Coordination specific projects including developing training and data collection tools to measure outcomes.
  • Independently track and manage the data collection tools and daily work books for Care Coordinators Efficiently multi-task numerous projects simultaneously. See above
  • Format various Microsoft Office documents
  • Oversee tracking and dispersion of payments for post-acute indigent patient services Collect requested information as directed
  • Collate and report on data from multiple software by running system reports (Canopy, Crimson, Premier, Star, EPIC, etc.) Provide timely and accurate administrative support services
  • Schedule meetings, meeting preparation (send out reminders, packets, set-up equipment, minutes)
  • Be timely and responsive when dealing with unscheduled requests
  • Assist with developing presentations and meeting materials Ensure quality customer service with an understanding and awareness for specific needs, chain of command and interpersonal relationships
  • Display professional and courteous behavior in support of achieving organizational and departmental goals
  • Screen telephone calls and direct caller to appropriate party
  • Assist with orientation of new staff Performs other duties as assigned Complies with all WellStar Health System policies, standards of work, and code of conduct.
Required Minimum Education:

Four years of college Required or equivalent work experience. Required and Completion of a college level data mining/data management/data analysis class. Required and Additional training in personal computer applications and medical terminology Required Required Minimum License(s) and Certification(s): All certifications are required upon hire unless otherwise stated. Additional License(s) and Certification(s):

Required Minimum Experience:

Minimum 1 year administrative experience; performance improvement (PI) and healthcare administrative field Preferred and Minimum 1 year + experience writing SQL statements/queries, Required and Minimum 1 year + experience with MS Accessimporting spreadsheets, tables, Required and Minimum 1 year + experience working with spreadsheets and/or the completion of an accredited college course in data mining/management.

Required Required Minimum Skills:

Expert working knowledge of MS Excel, Microsoft office including word, PowerPoint, Visio, etc. Strong word processing skills; spreadsheet design and management; presentations and graphics applications. Problem solving, and analytical skills. Self-directed, ability to work autonomously and prioritize multiple assignments without direct supervision. Very strong organizational skills. Strong communication skills both written and verbal. Ability to work collaboratively with and through other people. Familiarity with the management and PI tools utilized within the WellStar.
