Southeast. Always the right career direction. Job Description Summary The Stroke Coordinator assists with the promotion and growth of the Acute Stroke initiative with our affiliate hospitals. The coordinator is responsible for the implementation and maintenance of the Stroke Network Program. They serve as a clinical role model, educator, and liaison for Southeast Health Stroke Center, hospital staff and community. Collaborates with the medical director and nursing leadership to facilitate and promote improved stroke patient outcomes. The Stroke Coordinator serves as the facilitator of quality assurance for the program as well as the data reporter. Stroke Coordinator will assist the affiliate hospitals in development of Stroke Programs/protocols within the Emergency Department and the community.

Job Description Essential Functions:

+ Takes ownership of the ongoing management of the stroke program across the entire patient journey, from initial admission through discharge and beyond, ensuring seamless care transitions. + Guarantees adherence to DNV standards to maintain Primary or Comprehensive Stroke Center certification, including the timely completion and submission of all required intra-cycle reports to DNV. + Develops, implements, and annually reviews clinical protocols, order sets, and care paths, ensuring they reflect the most current evidence-based practices and guidelines. + Maintains an accurate and up-to-date stroke registry through continuous and concurrent data collection. + Interprets stroke registry data, identifies trends, and presents findings to various committees and stakeholders to inform practice and improvement efforts. + Analyzes and interprets clinical outcome data to identify trends, develop insights, and make evidence-based recommendations for continuous quality improvement initiatives. + Serves as a clinical expert and consultant for complex and highly specialized stroke patient care situations, providing guidance and support to hospital staff, personnel, and other healthcare professionals within the organization and the community. + Manages the scheduling, technology, and delivery of remote stroke care services for patients and providers. + Promotes and facilitates the personal and professional growth and development of self and all patient care staff involved in stroke care through education and mentorship. + Establishes and nurtures collaborative working relationships with physicians involved in the care of stroke patients, providing leadership in facilitating effective communication systems among physicians and other healthcare team members. + Facilitates, supports, and co-leads the Stroke Team committee in partnership with the Medical Director, driving program initiatives and improvements. + Develops, monitors, and ensures adherence to the Stroke program’s financial plan. + Collaborates on and supports the execution of stroke-related research protocols and data collection. + Creates and implements initiatives to increase community awareness of stroke prevention, recognition, and treatment. + Oversees the work of assigned personnel within the stroke program, providing guidance, mentorship, and ensuring adherence to program goals and standards. + Conducts regular performance reviews for assigned team members, addressing discrepancies and providing constructive feedback to foster professional growth and ensure optimal team performance. + Utilizes effective verbal and written communication skills and maintains positive and professional interpersonal relationships with all stakeholders. + Represents the organization and stroke program at local, state, and national meetings through educational presentations and active involvement in relevant professional organizations. + Performs other duties as requested by primary manager that do not compromise moral code of conduct or protocols set in place for patient or employee safety .

Qualifications:
Education Required:

+ Must possess a current Registered Nurse (RN) license in the State of Alabama or hold a multistate license through the Nurse Licensure Compact (NLC) or enhanced Nurse Licensure Compact (eNLC). + Stroke Certified Registered Nurse (SCRN) certification OR an ASC – Advanced Stroke Coordinator certification within two years of the hire date or internal transfer. + National Institutes of Health Stroke Scale (NIHSS) certification is required within ninety (90) days of employment, along with demonstrated continuous competency.

Experience Required:

+ Three (3) to five (5) years of clinical experience (Medical-Surgical, Critical Care or Emergency Department). + Basic Life Support (BLS)

Education Preferred:

+ Academic preparation as an advanced practice nurse or American Board of Neuroscience Nurses- Certified + MSN or related Master’s degree preferred .

Experience Preferred:

+ Academic preparation as an advanced practice nurse. + Teaching experience in the clinical and/or community setting preferred. + Experience in data collection and analysis preferred. + Advanced Cardiac Life Support (ACLS). Required Knowledge/ Skills/ Abilities + Possesses the advanced clinical skills necessary to expertly conduct comprehensive stroke assessments utilizing tools such as the National Institutes of Health Stroke Scale (NIHSS) and other sophisticated neurological evaluation methods. + Demonstrates skill in applying an in-depth understanding of complex stroke management principles, including acute interventions, secondary prevention strategies, and rehabilitation protocols. + Exhibits highly developed critical thinking and problem-solving skills to effectively address the multifaceted challenges inherent in stroke care coordination. + Possesses strategic project management skills essential for planning, implementing, and evaluating stroke program initiatives. + Demonstrates skill in advanced data management and statistical analysis to interpret stroke registry data and drive quality improvement efforts. + Exhibits skill in the proficient development of evidence-based clinical policies and protocols. + Demonstrates skill in ensuring comprehensive regulatory compliance with standards set by DNV (or other accrediting bodies) and the State of Alabama. + Possesses skill in fiscal responsibility and budget management, as applicable to the role’s responsibilities. + Demonstrates highly effective communication skills, both verbally and in writing, to interact with diverse audiences. + Exhibits exceptional collaboration and team leadership skills in fostering a cohesive multidisciplinary team environment. + Possesses skilled facilitation and conflict resolution abilities to navigate complex team dynamics. + Possesses skill in the advanced utilization of electronic health record (EHR) systems for efficient documentation and communication. + Demonstrates mastery in the use of stroke registry software for accurate data management and reporting. + Exhibits skill in the proficient use of the Microsoft Office Suite for various program-related tasks. + Has the ability to function autonomously and proactively address intricate clinical and programmatic challenges. + Possesses the ability to conduct both formal and informal research methodologies to advance stroke care practices. + Demonstrates the ability to adapt to a highly adaptable and responsive work ethic, including flexible scheduling. + Demonstrates commitment to organizations five (5) priorities and Six Ground Rules + Person in this position is required to understand, agree upon and follow our
