As part of our Adult Community Services department, you will play a vital role in connecting people with essential resources, services, and support. We’re seeking a dedicated Outreach Specialist to work with our PATH Grant program, focusing on Adult ERE Outreach and Youth ERE Outreach. As an Outreach Specialist, you will provide short-term case management, connect individuals with community resources, and offer support to those in crisis. Key Responsibilities:

Engage and serve individuals who utilize emergency rooms and hospitals frequently due to unmet behavioral health needs Provide short-term case management and connect people with community resources Offer support to those in crisis, including youth and families Develop and maintain positive partnerships with hospitals, community mental health providers, and substance use treatment providers Conduct outreach services via community locations, homeless camps, and street outreach Provide individual therapy to youth with identified behavioral problems and functional deficits Assist persons served in identifying appropriate resources and developing ongoing community support systems Education, Experience, and/or

Credential Qualifications:

Advanced knowledge in mental health issues as evidenced by a bachelor’s degree in Psychology, Sociology, Social work, or other related human service specialized fields of study, including a BHS degree from an accredited institution Valid Driver’s License required Auto liability insurance, including uninsured motorist coverage, required

Additional Qualifications:
