Summary:
Responsible to CSBG Administrative Assistant/Emergency Services Program Coordinator for client services, including outreach, intake, referral and case-management for program participants of the Alabama Council on Human Relations, Inc. Work closely with other agency programs. See that clients are assessed to determine needs; work to support clients in setting goals and finding support for those needs so that households become self-sufficient. Responsible for displaying appropriate work habits, keeping necessary records, participating in training and upholding agency policies, practices and procedures.
Primary duties:
Work with clients through intake, outreach and referral to help them find solutions to problems in areas of housing, health, and other needed services; document all contacts using the FACSPro Tracking System. Complete Matrix and follow-ups, etc., documenting all contacts and helping clients set goals, both long and short-term, and work toward meeting them. Serve as advocate for and liaison between clients and community resources. Serve as catalyst to facilitate action in empowering clients to become active advocates for themselves and their children. Work with ACHR staff components as agreed on with CSBG Administrative Assistant/Emergency Services Program Coordinator to develop and conduct needed training sessions for individuals or groups. Keep up-to-date records/documentation on all clients in caseload. Give supervisor weekly updates of clients on caseload. Learn about & how/when to serve clients under Low Income Home Energy Assistance Program. (LIHEAP) and manage time between

workload as directed by CSBG Administrative Assistant/Emergency Services Program Coordinator. Learn and assist with other emergency services-related programs as requested. On behalf of

clients, deliver necessary checks to vendors in a timely manner directed by the CSBG Administrative Assistant/Emergency Services Program Coordinator. Complete Matrix on each client in caseload and on other persons as requested by CSBG Admin. Asst./EMS Coordinator. Do and document periodic follow-up as needed and required to ensure clients are meeting or putting forth efforts to meet goals -as agreed on in Matrix and to ensure services are delivered or accessed and if not, reason is documented in FACSPro. Assist clients in meeting established goals through identification of resources, advocacy, referrals and provision of other needed services. Document contacts, problems, and progress routinely in FACSPro. Identify needs and problems as work with clients and make appropriate referrals for health, social, economic, or other problems affecting each client. Referrals may be made to external sources or to services within ACHR. Maintain (as per FACSPro Tracking System) all necessary records for clients assigned to caseload. Keep up-to-date records on all referrals. Keep

files in designated file cabinets. Read and follow guidelines in ACHR, Inc. Policy and Procedure Manual, assigned sections of ACHR, Inc. Procedure Manuals, CSBG and LIHEAP Procedures, FACSPro Tracking System, as well as other program guidelines. Read and understand ACHR goals, philosophy, policies, and procedures and act on same. Attend staff meetings as assigned. Attend trainings as assigned, (webinar, local and/or out-of-town/overnight). Work steadily to obtain and then maintain certifications as requested by supervisor or CEO. Maintain confidentiality.
