SUMMARY We are looking for staff to provide direct care to persons with developmental and physical disabilities in our Residential Group Homes. This person will be responsible for implementing behavior programs and other habilitation programs, and also facilitating and promoting community integration and safety.

ESSENTIAL DUTIES AND RESPONSIBILITIES

Ensure individuals’ medical health issues are addressed. This may include scheduling doctor’s appointments, providing transportation to appointments, following through on doctor’s orders and communicating all pertinent health issues to supervisor. Assist individuals in daily activities including daily hygiene functions. Implement behavioral goals, teaching strategies, monitoring procedures, exercise programs and other team recommended programs and procedures. Assist in the completion of individual assessments. Model positive adult roles in the community Foster community belonging through person-centered appropriate activities Asks for assistance from co-workers and supervisors when unsure of job duties and refers to program policies and procedures for job clarification. Supervises and assists in the preparation of food for individuals following special prescribed diets and food consistency. Implements eating procedures as written, if applicable. Overnight awake staff is required to complete hourly safety checks on the individual Assis individuals in community integration and improving their overall quality of life.

COMPETENCIES

Understanding the nature of human systems: individual, group, organization, community and society, and their major interactions for individuals with developmental disabilities. Skill in identifying and selecting natural opportunities which promote growth and goal attainment for individuals with developmental disabilities. Skill in implementing interventions for individuals with developmental disabilities. Process skills which are required to implement services for individuals with developmental disabilities. EDUCATION and/or

EXPERIENCE

High school diploma or general education degree (GED); 3 to 6 months-related experience and /or training, or human service training certificate.

CERTIFICATES, LICENSES, REGISTRATIONS

Valid Connecticut Driver’s License Ability to obtain the following licenses/certifications: Department of Developmental Services – Medication Certification (to be obtained within six months of hire) Public Service License valid for the state of Connecticut (if applicable) CPR and First Aid certification
