Job Summary :

The Client Engagement & Telehealth Program Coordinator plays a critical role in the development and maintenance of client care programs, ensuring successful patient navigation and program efficacy. This position manages client caseloads, coordinates enrollment into insurance premium assistance and telemedicine care programs and ensures smooth transitions across services. The role involves interdepartmental collaboration and external partners to address the evolving needs of clients, while also supporting grant and donor efforts for program growth.

Essential Job Functions:
Client Engagement :

Manage client caseloads, ensuring a seamless transition from outreach to intake or referral. Monitor client progress and maintain engagement with services.

Telemedicine HIV Care Program Management :
  • Evaluate clients’ eligibility and preparedness for HIV care through the telemedicine program.
  • Support clients living with HIV in transitioning between telehealth and in-person care, based on their evolving needs.
  • Participate in weekly meetings with the Telehealth HIV Program team to coordinate and address client care needs.
Insurance Premium Assistance Management :
  • Work with Insurance provider to find Affordable Healthcare Act (ACA) plans for clients during enrollment periods.
  • Ensure eligible clients remain enrolled and engaged in the insurance premium assistance program.
  • Assess eligibility for special enrollment periods and assist with client enrollment.
  • Address client concerns regarding insurance enrollment and maintain communication regarding eligibility.
Testing & Outreach :
  • Successfully adhere to a required monthly minimum of rapid HIV tests
  • Attend in-person outreach events as needed
  • Collaborate with Director of Outreach & Navigation to ensure compliance with local, state, and federal regulations
Program Design Collaboration and Implementation:
  • Propose and implement initiatives to improve the client care experience.
  • Identify and implement system improvements for tracking, client care workflows, and program delivery.
  • Collaborate with Director of Outreach and Navigation to ensure smooth integration of services.

Collaboration with

Healthcare Providers :

Coordinate engagement with medical providers, ensuring clients are connected to the right resources.

Resource Management :

Oversee department resources, vendors, technology, and supplies to ensure smooth program operations.

Administrative Support :

Provide support for the Executive Team and ensure smooth administrative processes for the program.

Essential Qualifications and Skills:
  • Commitment to collaboration and teamwork
  • Fluency in English and Spanish, with strong written and verbal communication skills.
  • Bachelor’s degree (or three years of relevant work experience).
  • Passion for PrEP and familiarity with HIV prevention, prescription processes, and clinical guidelines.
  • Culturally competent in LGBTQ+ life and culture, with sensitivity to social justice issues, particularly within immigrant communities.
  • Familiarity with dating apps used by MSM (Grindr, Scruff, Sniffies, etc.).
  • Familiarity with major social media platforms (Instagram, Facebook,
  • Excellent organizational and time-management skills, with the ability to manage multiple priorities.
  • Technology-savvy, with proficiency in mobile platforms and major computer applications.
  • Experience with data collection and database systems.
Preferred Skills and Experience:
  • Sales or similar experience.
  • Experience with electronic medical records systems.
  • Ability to troubleshoot common computing technical issues.
  • Two or more years’ experience in LGBT community-based organizations or healthcare settings.
  • Florida certified HIV tester (500/501).
  • Basic understanding of HIPAA and client data privacy requirements.
  • Experience with or understanding of the 304B pharmacy program.
