Job Summary:

The Resident Services Coordinator will provide case management in a community based model to improve educational outcomes and intergenerational mobility for youth with services and support delivered directly to households. Essential Duties/Expectations:

Engage with residents to collaboratively complete the initial assessment/survey Work collaboratively with the POAH Communities’ Community Impact and Property Management teams to offer housing stability support to Cutler Manor families Provide services utilizing evidence-based practice in service delivery such as case management, Motivational Interviewing, stages of change, Harm Reduction, Trauma Informed Care, Critical Time Intervention and Housing First Practices to assist in obtaining/increasing income, promoting self-sufficiency and housing stability Work with residents on an individualized services plan (ISP) Coordinate with community providers to offer additional services in the areas of, but not limited to: housing stabilization, money management, community integration, employment and training, benefits establishment, referrals to community providers for substance use, primary and mental health care, and all other services needed to assist client in reaching their goals Conduct home visits with each resident. Support housing stability efforts through referral coordination and relocation assistance in partnership with POAH Community Impact Team Maintain all client records and information in accordance with the CHOICE grant data collection (Salesforce)/ tracking policies and procedures Ensure compliance with funding sources and timely data entry into Salesforce or any database as defined by the CHOICE grant Complete all documentation, paperwork in a timely and efficient manner • Actively participate in weekly/monthly supervision Attend scheduled workshops, trainings and meetings as required Cross train across all programs/departments to ensure success of Carrfour Supportive Services Provides support to families during emergency situations as defined by the

Ability to travel throughout the neighborhood and engage in door knocking efforts Ability to work periodic overtime and flexible hours required, including weekends and evenings

Minimum Requirements:
Education/Experience:

High School Diploma or GED required, Higher education in social work, mental health, psychology, or related field preferred but not required Experience working directly with individuals from diverse racial, ethnic, and socioeconomic backgrounds required Individuals with less than five years related field experience will be required to provide proof of certification (mental health or behavioral) or education. Experience working with evidence-based techniques preferred (Trauma Informed Care, Motivational Interviewing, Harm Reduction, Crisis Intervention, etc.) Previous experience in affordable housing preferred Knowledge of community resources Valid Florida driver’s license and car required Ability to work a flexible schedule and be on-call as needed
