Role:
We are seeking a dedicated LTSS Service Coordinator to support individuals in specialized programs, ensuring their physical and behavioral health needs are met through a person-centered care approach. You will facilitate service coordination, assessments, and care planning, working collaboratively with medical directors, interdisciplinary teams, and the member’s support circle.
Key Responsibilities:
Conduct face-to-face assessments using structured tools and motivational interviewing techniques. Develop and manage a Person-Centered Support Plan (PCSP) aligned with the member’s preferences, goals, and needs. Serve as the single point of contact for members, coordinating non-clinical needs for individuals with chronic illnesses, disabilities, or co-morbidities. Submit utilization/authorization requests and ensure compliance with state and federal regulations. Report critical incidents to appropriate agencies, including Adult Protective Services and Law Enforcement. Support appeals, grievances, and audits as needed. May mentor new staff and participate in process improvement initiatives.
Qualifications:
BA/BS degree in a healthcare-related field preferred. Minimum 2 years of experience working with a social work agency (or equivalent combination of education & experience). Case management experience preferred. Strong organizational, time management, and interpersonal skills. Adaptability to multiple computer systems. Bilingual candidates preferred.
