Job Description

Memory Enhancement Program Facilitator, Fruit Cove, St Johns FLorida St. Johns County Council on Aging, Inc. 501 State Road 13 North, Fruit Cove, FL 32259 Part-time, 6 hours per week. Thursday 8:30 a.m. to 2:30 p.m. ; Substitute on other days when needed Facilitates all components of the Memory Enhancement Program (IMEP) including therapeutic group activities, daily operating management, maintenance of program and participant records, and supervision of volunteers. Interacts with and responds appropriately to the needs of program participants and caregivers. Essential Duties and Responsibilities for

IMEP Facilitator:

Implement all components of IMEP including activities, exercises, daily operating management and maintenance of all program and participant records. Be an ambassador to the IMEP program both inside and outside the classroom. Interact with, be sensitive to, and respond to the needs of participants and family caregivers. Understand the needs of the elderly and in particular the needs of those affected by memory loss. Be willing to substitute for other facilitators during absence and occasionally work at other IMEP locations when needed. Adhere to the IMEP Daily Agenda including all therapeutic components, exercises, and activities as scheduled, unless group needs necessitate a schedule change or additional activities. Ensure that at least two staff or one staff and one trained volunteer are present during all hours of program operation. Contact IMEP Manager regarding any problems, questions, concerns, or emergency situation. Provide observation, monitoring and intervention if unstable mental status or medical condition arises. Ability to provide basic First Aid and when deemed necessary, call 911. Notify IMEP Manager after calling 911. Notify participant’s emergency contact after calling 911. Monitor the provision of meals and participants dietary and fluid intake. Ensure participants remain safe and do not wander outside of the building. Work as a team with other staff and volunteers in presenting the daily program and activities. Attend IMEP staff meetings as scheduled. Comply with all HIPAA privacy laws and all COA policies and procedures. Ensure that all program paperwork including sign in sheets, time sheets, client information forms, and emergency contact information forms are recorded and accurate. Perform related duties as assigned by supervisor.

Qualifications:

Prior experience in small group facilitation and/or education. Knowledge of early memory loss/early dementia is helpful. Must pass AHCA Level 2 background screening requirements. Be trained or be willing to be trained in CPR/AED and First Aid. Ability to work independently in an off-site location Excellent verbal and written communication skills; ability to effectively communicate with clients. Excellent computer skills in MS Office (Word and Excel), and G Suite Tools (Gmail, Drive, Sheets, and Calendar). Ability to learn computer software programs related to client assessments and services. Must be able to work under pressure and meet deadlines while maintaining a positive attitude High School Diploma/GED Required; Bachelor’s Degree in Nursing, Psychology, or Education preferred.
