SUMMARY :

The Assistant Program Manager (APM) is a member of the Fulton Permanent Supportive Housing (PSH) program and responsible for the daily program operations of the program in conjunction with the Program Manager. The APM leads, administers, and oversees program services and team members who assume responsibility for providing direct, onsite services to adults in the PSH program. Fulton PSH services include the following services: assessment and intake, case management, care coordination, psychosocial rehabilitation, and crisis intervention.

DUTIES :

The following reflects essential functions for this job but does not restrict other tasks which may be assigned: Leadership, Administration, Oversight Support Fulton PSH staff in completing daily, weekly and monthly tasks required for program operations. Train new staff on Step Up policies and Fulton PSH Program Overview and Guides. Assume leadership on the Fulton PSH team when Program Manager is unavailable. Attend meetings with various stakeholders at the request of the Program Manager. Collaborate with Director and Program Manager as requested to assist with staff recruiting and onboarding, including attending job fairs, screening potential applicants, and coordinating interviews and applications. Conduct staff meetings and supervision as requested. Provides on call support as needed. Acts as liaison with outside agencies to coordinate referrals and build community relationships. Direct Service and Support Conduct regular on-site visits at the various PSH properties to ensure contract deliverables are being upheld and Property Management has an open line of communication and support. Engage community partners in the City of Atlanta and Fulton County to increase client and staff support. This includes meeting with community partners to develop resource agreements and seeking out agencies to fill service gaps. Conduct staff supervision and engage in staffing challenging cases. Assist in developing and implementing staff training. Provide access to training/learning opportunities internally or in the community. Provide coverage by providing staff support, client support and crisis intervention, as needed. Documentation and Data Collection Monitor electronic and paper documentation and member files to maintain compliance with HIPAA, Step Up and Fulton Behavioral Health Network requirements. Review notes and documentation for assigned staff for quality of submission and assist staff with meeting productivity expectations. In collaboration with the Program Manager, work with staff to collect data specific to the Fulton PSH contract and KPI. This includes reviewing ClientTrack HMIS and Welligent for accurate notes, community referrals and incidents. Review staff documentation and client charts and report findings to Program Manager with concerns or highlight areas of improvement to ensure file compliance. Authorize or co-sign paperwork needed.

SKILLS :

Leadership, motivation, and management experience preferred; strong clinical assessment, interpersonal, and intervention skills required. Accuracy in charting of required documentation. Ability to train or guide staff to achieve confidence and ability to work independently. Strong organizational skills and ability to delegate tasks to staff and monitor outcomes. Experience with Excel and ClientTrack HMIS. Experience conducting clinical assessments.

QUALIFICATIONS

: Master’s degree in human services, counseling or related field. Three years’ experience in community-based work. Three years’ experience in case management or direct clinical services. Experience working with adults with SPMI. Experience working with the chronically homeless population. Case management experience. The candidate must have own transportation and current GA driver’s license and insurance. The position requires coverage for evenings and weekends for after-hours emergencies, on a rotating schedule. Strong computer skills required.

SUPPLEMENTAL INFORMATION

: Work with people experiencing homelessness whether on the streets, in shelters or other places of habitation or services, may present inherent challenges and difficulties such as: exposure to bed bugs or other infestations, unpleasant smells or odors, individuals who may have poor hygiene or unclean homes due to homelessness, mental health symptoms, or poverty. Additionally, in working with our members, employees may encounter instances of profanity, sexually explicit or derogatory language, or verbal or physical expressions of anger and trauma. Very rarely do these situations ever escalate to physical contact, and negative or derogatory communication patterns can often be negotiated successfully. All direct care staff are provided adequate training to develop skills to deescalate crisis situations that may arise and Step Up has established procedural safeguards for all employees to mitigate (but not eliminate) these inherent risks of employment.

PHYSICAL REQUIREMENTS

While performing the duties of this job, the employee is regularly required to sit; use hands and fingers; handle or feel; reach with hands and arms; talk; and hear. The employee is frequently required to walk, balance, stoop, kneel, and/or crouch. The employee must occasionally lift and/or move up to 15 pounds. Specific vision abilities required by this job include close vision, distance vision, peripheral vision, depth perception, and ability to adjust focus. Keyboard data entry required.
