Job Description Integral to the team, the Director of Care Management will handle a wide range of projects to support and implement the organization’s model of clinical care, driving initial market workflows and infrastructure development. This includes leading the design and rollout of care management processes, collaborating with cross-functional stakeholders to ensure operational readiness. This role will also play a key role in leveraging data and analytics to inform program strategy, guide resource allocation, monitor performance metrics, and identify opportunities for continuous improvement across the Care Management team. How you can make a difference Develop and lead care management strategies that support the goals of value-based care to drive transformation in care delivery by integrating clinical, behavioral and social determinants of health to advance patient-centered care and operational effectiveness Actively engage with both internal and external stakeholders—including primary care providers, community partners, provider networks, and healthcare collaborators—to extend the organization’s impact and ensure continuity of care across settings Hire, train, supervise, and mentor care management teams across multiple sites or regions, provide clinical guidance and decision-making support to care team members handling complex patient needs Set clear expectations and performance standards for care management staff, holding team members accountable for quality care, timeliness, and effectiveness of interventions Promote evidence-based practices and clinical protocols to ensure safe, high-quality, and patient-centered care across all care management activities Leverage data analytics to drive improvement by partnering closely with analytics team to define, interpret and utilize actionable data. Work collaboratively to build and refine dashboards and reporting packages to support clinical and operational opportunities Serve as a thought leader in care management innovation, quality improvement, and population health strategies to articulate challenges and identify solutions that contribute to the development of a sustainable program Create a culture of clinical excellence and continuous learning by leading by example through transparency, integrity, and responsiveness. Create an environment where feedback is welcomed, and staff are empowered to innovate and excel Skills for success Deep understanding of care management principles, risk stratification models, and social determinants of health Knowledge of chronic disease management, behavioral health integration, and transitions of care In-depth understanding of value-based payment models (e.g., MSSP, ACOs, Medicare Advantage, shared savings) Strategic ability with an affinity for operations; not afraid to dig into the details of the work to influence a positive outcome Ability to create structured narrative around qualitative data Superior relationship skills with the ability to craft meaningful partnerships Comfort with conflict resolution, change management, and driving consensus across diverse stakeholders Positive attitude with a solution-oriented approach and a comfort working within ambiguity A low ego and humility; an ability to gain trust through communication and doing what you say you will do Experience to drive change Bachelor’s degree in nursing with current RN licensure in state of Georgia, OR Master’s degree in social work with clinical license in state of Georgia Minimum 5 years’ experience in Care Management, with at least 3 years in a leadership role Experience working in a value-based care environment Proven success in managing multi-disciplinary teams and driving clinical performance outcomes In-depth, working knowledge of state and regulatory guidelines Demonstrated business and financial acumen Proficiency in the Microsoft office suite of products including Outlook, Word, Excel and Power Point Proficiency in EMRs and care management software programs About Our Commitment Total Rewards at VillageMD Our team members are essential to our mission to reshape healthcare through the power of connection.
