JOB DESCRIPTION
The Family Support Specialist assists the Family Resource Center Program Manager in providing Prevention Services and Referral Assistance to at-risk families in Fayette and Spalding Counties.
GOALS AND OBJECTIVES
Strengthen parental capacity to prevent maltreatment, out-of-home placement, and Division of Family and Children Services Intervention.
PRIMARY RESPONSIBILITIES
Conduct a variety of outreach activities and culturally sensitive events, including group presentations, networking, and attendance at community events to introduce the FRC program to potential clients. Provide family support and case management (meeting with youth and/or families in the community and/or in the home). Initiate and maintain weekly, bi-weekly, and/or monthly follow-ups for ongoing case management. Participate in developing Individual Family Support Plans to support FRC participants with goal setting and establish intervention services. Maintain a caseload of up to 40 families. Disseminate program information at events/activities to educate families/communities about the long-term benefits of available FRC programs/services. Identify members of the applicable service population in the targeted community and motivate them to participate in applicable FRC Services. Develop key community relationships throughout the targeted area to enhance outreach and increase the number of client referrals. Solicit volunteers through contacts in the community. Complete Triple P (evidence-based Positive Parenting Program) training and certification; implement as directed. Provide referrals and assistance with obtaining additional resources including food, WIC, SNAP, Medicaid, and other concrete supports. Complete forms, reports, and required program documentation by the provided deadlines. Develop a strong knowledge base and stay current on job-related issues and trends. Participate actively in departmental meetings, training, and education, and the quality process. Comply with Bloom’s, policies, procedures, and all state regulations. Willingness to work in high-risk neighborhoods. Remain aware of social, economic, cultural, and environmental factors in resolving family and personal problems. Interact effectively with children/families from diverse backgrounds. Perform under strong demands in fast-paced, diverse, sometimes ambiguous environments. Commit to providing high customer satisfaction with positive service delivery results. Meet critical deadlines while maintaining attention to detail, accuracy, and quality. Handle highly stressful, sensitive situations; maintain confidentiality and professional boundaries. GENERAL EXPECTATIONS
This job operates in a professional office environment. This is a full-time position requiring flexibility. Some weekends and nights may be required. Travel throughout the south metro Atlanta area, Fayette County, Spalding County, and surrounding areas.
