Primary duties include developing relationships with the clients and families on their case load, using person-centered planning techniques to help clients set and achieve realistic goals related to community living, monitoring, and supporting success in daily routines, and teaching daily living skills via the My Full Life platform. Responsible for designing and implementing learning opportunities in their specialty area. Required Qualifications Bachelor’s degree preferred, in social work or related human services field with at least 2 years’ experience in a social services capacity. Preferably someone with experience working with people with a disability, experience liaising and networking with a variety of agencies, and existing knowledge of professional ethics working with people with a disability. Must be at least 21 years of age, have a valid Class C Georgia Driver’s License, proof of vehicle insurance, and meet company policy regarding MVR requirements. Must meet state requirements regarding a GBI and FBI criminal history record check which requires fingerprinting. Must undergo and pass a drug screening and screening for Tuberculosis. Major Duties and Responsibilities Develop a relationship with clients and families emphasizing open communication, consistency, and trust. Facilitate the development and implementation of a Personal Achievement Plan designed to identify values, interests and strengths and establish realistic and achievable goals related to community living. This includes periodic plan modifications. Evaluate and document progress in short- and long-term goals in client files. Collaborate with Coaches to ensure proper translation of independent living goals into daily routine. Assists with securing appropriate housing, compatible roommates, and overseeing superintendent/landlord relations. Establishes professional services as needed. Develop daily system of check-ins and weekly meetings with clients and coaches to monitor general health and safety. Work with coaches to establish emergency procedures and periodically reinforce skills and associated plans. Assist clients in developing vocational skills, exploring job opportunities, and applying to local jobs. Work with Specialists to develop an informal support network for clients, including local family, friends, and community members willing to support their journey to independence. Specialists should be a leader in their field of expertise (ex. vocational support, community relationships and access, daily living skills, etc.), and should continue to seek training opportunities to remain best in class. May perform duties and responsibilities of Independent Living Program coach to supplement program staffing. Participate in the teaching of daily living skills via the My Full Life platform. Act as an advocate for all clients with their families, other staff, and providers. Perform other duties as assigned that may be necessary in the best interest of Annandale at Suwannee, Inc. to meet the patient care needs of the Villagers. Employee may be required to transport (drive) and assist Villagers in and out of vehicles. Employee is required to perform CPR/First Aid.
