Job Description
Within the Advanced Heart Failure Program at Beth Israel DeaconessMedical Center, the Heart Transplant Practice Specialist willcollaborate with the nurses, physicians, pharmacists, and nursepractitioners of the program to assist in the safe delivery of patientcare. Additional support staff of the Heart Transplant departmentincludes administrative assistants, data coordinators, and financialcoordinators. The multi-disciplinary approach used for patients duringthe evaluation, pre-operative, and post-transplant phases of care helpsto ensure that all groups are involved throughout the heart transplantprocess. In 2024, the Advanced Heart Failure Program will move into anew ambulatory space that will include clinic space for patientevaluation as well as administrative offices for the multi-disciplinaryteam members.
Essential Responsibilities:
Obtains referrals required forpatient prior to scheduled visits; maintains 100% compliance withreferral management requirements and current knowledge base of referraland authorization rules and related policies for multiple insurance andmanaged care payers including transplant specific policies; maintainsrecord of referral, level of care and number of visits being authorized.
Assists patients with full registration, updates insurance informationat each visit to ensure collection of accurate information for billingand accurate capture of pre-transplant services; collaborates withFinancial Coordinator to verify that mandatory policies are followed;notifies Financial Coordinator of insurance changes identified atcheck-in. Performs check in, and coordinates and schedules multiple newand follow-up appointments in person, via e-mail and over the phone tosupport a multidisciplinary team including surgeons, hepatologists,nephrologists, infectious diseases, social workers and nutritionist.
Confirms correct insurance type, type of visit and demographicinformation. Collaborates with Transplant Nurse Care Coordinators tomaintain up-to-date patient information in the Organ Transplant andTracking Record database and records time per instructions on monthlyMedicare time study. Answers, screens and processes a high volume ofcalls in a professional manner; utilizes and adheres to the phonescripts and guidelines for triaging calls; asks appropriate questionsand uses independent judgment within scope of knowledge and authority todetermine the type of appointment, appropriate provider and urgencyneeded. Records and forwards accurate messages to providers and staff.
Required Qualifications:
High School diploma or GED required.
Associate’s degree preferred. 1-3 years related work experiencerequired. Experience with computer systems required, including web basedapplications and some Microsoft Office applications which may includeOutlook, Word, Excel, PowerPoint or Access. Experience in healthcarestrongly preferred.
