PRIMARY RESPONSIBILITIES

Assist participants with IDD and/or Autism in developing goals and strategies, which best service each individual according to their Individual Support Plan (ISP). Manages and advocates for a caseload of participants and directly supervises all Direct Support Staff. Trains Direct Support Staff on agency policy, the implementation of ISPs, and on behavioral management techniques. Assists the consumer with attaining goals in order to have positive outcomes as described in the individual’s ISP. Monitor individuals’ progress and service delivery through on-site visits and data collection. Travels to and participates in monthly team meetings, ISP meetings, Annual Review Updates, and other team meetings as necessary. Respects the autonomy and right to confidentiality of all consumers and families. Produces accurate documentation of the services. Documents care provided and response of the consumer/family. Submits time sheets and all other documentation in a timely manner. Advocates for changes in service(s) as necessary. Provides coverage in emergency and/or crisis situations. Performs all duties in an ethical manner and in compliance with local, State and Federal laws, Medicare/Medicaid regulations and established personnel policies and procedures, including confidentiality and HIPAA guidelines. Performs other job-related duties as may be assigned. Knowledge and Experience. Bachelor degree and 3 years of experience in the human services field. .
