Individual Care and Service Coordination Assessment and Care Planning:

Conduct individual assessments to determine eligibility and need for Home- and Community-Based Services (HCBS), including personal care, nursing care, respite, and environmental modifications. Develop person-centered care plans in accordance with DDS and MassHealth standards.

Service Coordination:

Coordinate and facilitate the delivery of services (e.g., personal care attendants, homemaker services, community habilitation, and skilled nursing) in collaboration with individuals and family members. Manage onboarding documentation, schedule coordination, staff meet-and-greets, and ongoing monitoring of service delivery.

Community Outreach and Networking:

Build and maintain relationships with DDS Service Coordinators, Area Offices, and other local organizations to inform them about available services and facilitate timely referrals.

Referral Generation:

Proactively generate referrals from community partners for individuals with IDD in need of services and supports. 2.

Referrals, Staffing Coordination and Onboarding Staffing Needs Assessment:

Collaborate with regional managers and team leads to assess staffing needs based on referrals and service demands.

Referral Process Management:

Manage and oversee the referral and intake process, ensuring accuracy and alignment with individual needs.

Candidate Evaluation:

Screen candidate resumes, conduct initial interviews, and assess qualifications relevant to the individual’s plan of care.

Facilitate Interviews & Onboarding:

Organize interviews and coordinate onboarding, including completion of all state-mandated documentation, CORI checks, and any DDS- or MassHealth-required training.

Training Needs:

Identify required certifications (e.g., CPR, First Aid, MAP) and coordinate timely completion. 3. Case Management Implement Individual Support Plans (ISPs): Collaborate with DDS Area Office staff, individuals, and families to implement and monitor ISPs and associated goals.

Monitor Service Delivery:

Ensure services are delivered as outlined in the ISP, in compliance with Massachusetts HCBS waiver guidelines.

Ongoing Assessment:

Reassess individuals regularly and update service plans as needs evolve. 4. Compliance and Documentation Oversight of Direct Support Professionals (DSPs): Monitor DSP documentation, including daily service notes, EVV (Electronic Visit Verification), and outcome-based reporting.

Regulatory Compliance:

Ensure all services adhere to DDS regulations, MassHealth standards (such as Adult Foster Care and Group Adult Foster Care guidelines), and applicable Medicaid rules.

Maintain Documentation:

Maintain accurate and complete documentation of assessments, ISPs, incident reports, and staff credentials.

Incident Reporting:

Report all incidents per DDS Incident Management Protocols and MassHealth guidelines. 5.

Advocacy and Individual Support Advocacy:

Advocate for individuals’ rights, ensuring informed choice, dignity, and inclusion in their communities.

Promote Independence:

Empower individuals to increase independence in daily living, employment, and social participation. 6.

Training and Staff Supervision Staff Training:

Provide training on person-centered planning, DDS/MassHealth documentation standards, and internal protocols.

Supervision:

Supervise DSPs to ensure high-quality service delivery aligned with the ISP, maintaining safety and dignity. Education Bachelor’s degree in Social Work, Psychology, Special Education, Human Services, or a related field preferred. Experience Minimum of 2 years of experience working with individuals with intellectual and developmental disabilities, ideally in case management, program coordination, or direct services. Familiarity with Massachusetts DDS and MassHealth waiver programs such as AFC (Adult Foster Care), PCA (Personal Care Attendant), and Shared Living. Knowledge of person-centered practices and behavioral supports is highly desirable. Clearance and Certification Requirements CORI (Criminal Offender Record Information) Check Motor Vehicle Record Check Massachusetts Child Abuse and Neglect Registry (if applicable) Authorization to work in the United States CPR/First Aid and MAP Certification (may be required depending on caseload) Reports to: IDD Team Lead Competency Requirements Strong written and verbal communication skills Working knowledge of DDS and MassHealth systems Ability to handle confidential/sensitive issues with professionalism and empathy Effective time management and organizational skills
