PRINCIPLE DUTIES/ESSENTIAL FUNCTIONS
:
- Assists with obtaining initial home care orders and F2F documentation
- Ensures referrals that are received are complete and include the reason for skilled care
- Manage referrals received both via phone and electronically from the receipt of the referral until the admission is scheduled
- Works closely with the field Clinical and Community Liaisons to track/monitor referral activity and sources, assists with scheduling on weekends as needed.
- Assists with following/tracking active clients that have been transferred inpatient
QUALIFICATIONS
:
- Medical background or Licensed LPN in Massachusetts
- 3-5 years’ relevant experience
- Detail oriented with exceptional customer service skills
- Computer proficiency is a must
