Summary:
The Resource Referral Specialist role provides information about the program to callers and walk-in inquiries and completes initial eligibility, screening, scheduling and referrals as appropriate. Assists with enrollment, re-enrollment, prescription assistance and care management.
Required Minimum Knowledge, Skills, and Abilities (KSAs)
Education:
Two years of advanced education and experience required.
License/Certifications:

Experience:
Two years of experience in social/human services, health care management or related field required.
