Videos Duties This Social Worker (Program Coordinor) position is within the Geriric and Extended Care Service the VA Maine Healthcare System, Togus locion. Major duties include but are not limited to: Participes in direct caregiver and Veteran contacts to identify and assess caregiver stress/burden and the development and applicion of directed clinical trement interventions. Advises and collabores with interdisciplinary teams troughout the medical center on caregiver issues. Crees educional tools, develops programs and implements training focused on specific caregiver needs/issues. Provides educion and in-service training to employees, Veterans, families and caregivers. Provides ongoing consultion and support to colleagues regarding caregiver support. Develops and facilites caregiver support groups (face-to-face or VANTS telephone line) and educional programs. Conducts caregiver and family clinical interventions to facilite the identificion of caregiver stress, psychiric illness, emotional distress and social needs as trement issues. Provides referrals or consults, or in some cases direct admission of a Veteran toother VA programs Serves as the major consultant for informion on negotiing the complex processes caregivers, Veterans and families encounter when interacting with government and community agencies. Maintains a knowledge base of current theory and trement modalities by reading professional journals, tending conferences, clinic seminars and teleconferences, and merials distributed through VA and prive sector sources. Collabores with the pient aligned care team, mental health and geriric primary care clinicians to provide Veteran and staff educion on caregiver issues. Coordinion will include hands-on support group work involvement, and training of pient aligned care team clinical social work staff in the development and facilition of such groups. Serves as a liaison between the VA and community agencies. Participes in caregiver support activities in the local community and ste to promote and recognize the role of the caregiver.
