Place Coordinator :
The Aging in Place Coordinator is responsible for providing case management, referrals, and support services to adults aged 55 and older, helping them maintain independence and quality of life in Boone County. This role involves conducting assessments, coordinating resources, advocating for seniors, and engaging in community outreach to connect individuals with essential services.

Recruit and screen consumers per program guidelines. Conduct intake and home assessments to identify safety concerns and needed services. Assist consumers in setting and monitoring goals for aging in place. Provide service coordination and referrals as needed. Refer consumers for in-house services (e.g., food pantry delivery, friendly visiting). Conduct annual re-assessments and evaluations. Educate consumers on their rights and independent living philosophy. Support consumers in developing self-advocacy skills. Collaborate with the Education Coordinator on programs enhancing older adults’ quality of life. Maintain accurate case notes and documentation in the database. Provide reports to management in a timely manner. Coordinate programs promoting independence (e.g., Property Tax Credit Program, Holiday Basket Program). Assist consumers in reporting suspected abuse, neglect, or exploitation and promptly report suspected cases to appropriate authorities. Conduct monthly community outreach to increase awareness of services. Collaborate with other agencies to minimize service duplication. Represent the organization at internal and external community events.
Position requirements:
A bachelor’s degree or higher in psychology, social work, or a human services field is preferred. Minimum two years of experience working in social services, or one year of direct experience working with seniors is required.
