Boone Health Stroke Program Overview Job Summary:

TCD Coordinator is focused on patient, program and community outcomes related to STEMI and Stroke. The coordinator collaborates with the STEMI and Stroke Center Medical Directors to ensure the delivery of appropriate clinical care to patients diagnosed with STEMI and Stroke. This is accomplished through the design and maintenance of a service delivery program congruent with evidence-based medicine, clinical practice and accreditation standards.

Job Responsibilities:

Provides project management support involving Time Critical Diagnosis initiatives for the STEMI and Stroke programs: Supports the education, coordination, and ongoing measurements of each program. Serves as a facilitator, resource for data compilation & analysis, and a consultant/educator on quality initiatives for each program, to both internal and external customers. Collaborates with key stakeholders for all time critical diagnosis regulatory initiatives for both program, to ensure compliance. Enters and abstracts clinical data accurately using indicators and/or standardized data collection tools. Gathers essential, relevant information on which to base improvements in quality, processes & functions and collaborates with key stakeholders for committee work involving these initiatives. Coordinates special/specific projects and programs as per department management. Types and distributes memos, correspondence & reports and composes documents as needed. Performs other responsibilities as assigned.

Nursing Diploma/Associate’s/Nursing Experience:
