Full Job Description Position Summary:
The DMH Service Coordinator plays a vital role in supporting Deer Valley’s Individual Supported Living (ISL) program. This position is responsible for ensuring the delivery of high-quality care and support to individuals with developmental disabilities in ISL homes. Serving as the primary liaison among clients, families, guardians, case managers, nursing staff, Direct Support Professionals (DSPs), and the Department of Mental Health (DMH), the Support Coordinator ensures all programmatic, medical, and administrative functions are carried out in alignment with state regulations and agency standards. This is a dynamic, hands-on role requiring flexibility, professionalism, strong communication, and exceptional time management.
Key Responsibilities:
Client Support & Oversight Maintain in-depth knowledge of each ISL client’s Individual Support Plan (ISP), Behavior Support Plan (BSP), and daily routines. Provide ongoing oversight of consumer health, medications, and overall quality of life. Schedule and attend all major medical appointments and ensure documentation is complete and up to date. Monitor medication changes and ensure timely updates to MARs and physician orders. Schedule annual ISP meetings, write plans, coordinate team participation, and manage required signatures and addendums. Ensure client files, binders, and documentation are current in both office and home. Staff Supervision & Training Train DSPs on ISPs, BSPs, medication procedures, and consumer-specific care. Provide written and verbal feedback to staff, including performance evaluations and support. Ensure all staff have completed and signed necessary documentation, such as ISP acknowledgment, medication side effects, and training forms. Maintain up-to-date staff training records and ensure continued compliance. Create and manage staff schedules to ensure full coverage according to ISL budget/staffing pattern. Administrative & Operational Duties File and organize all data sheets, logs, progress notes, and health records. Submit accurate and timely documentation, including incident reports, monthly reports, physician consultation forms, safety inspections, and financial records. Create and lead monthly staff meetings, maintain meeting minutes, and conduct follow-up trainings. Monitor ISL home conditions including cleanliness, safety drills, food supplies, and household necessities. Maintain and monitor emergency kits, first aid kits, and evacuation procedures at each home. Collaboration & Communication Serve as the central point of contact for families, guardians, case managers, pharmacy staff, and other stakeholders. Coordinate with nursing, HR, and finance departments to resolve issues and complete required documentation. Conduct at least two in-home visits weekly to review documentation, perform audits, and assess staff performance. Manage the on-call schedule and respond to emergencies during and after business hours. Compliance & Quality Assurance Assist with audit preparation and ensure homes are in compliance with DMH and agency standards. Maintain required documentation in accordance with DMH regulations and agency policies. Ensure financial accountability by balancing ledgers, submitting receipts, and managing personal and chaperone funds.
Required Qualifications:
Education:
Bachelor’s Degree in Special Education, Psychology, Sociology, Social Work, or related human services field.
Experience:
Minimum of 2 years working with individuals with developmental disabilities. Knowledge of Applied Behavior Analysis (ABA) principles preferred.
Certifications:
Must hold valid CPR certification prior to start. Must be certified in Medication Administration Level I, Mandt, and Behavior Management (or able to obtain upon hire).
Technology:
Proficient in Microsoft Office (Word, Excel, Outlook) and mobile apps. Comfortable with electronic documentation.
Physical Requirements:
Ability to perform moderate physical activity, travel between homes, and work flexible hours including evenings/weekends.
Travel:
Regular travel within the region required. Must possess a valid driver’s license, insurance, and reliable transportation.
Legal:
Must be registered with the Family Care Safety Registry and pass all required background checks.
