  • Support and assist priority populations at high risk of contracting
CCOVID-19

by reducing barriers to testing and services.

  • Work within their community and group class settings to help clients manage their own health including promoting compliance with
COVID-19

testing and potential quarantine/isolation.

  • Assists individuals and patients in navigating the health care system.
  • Facilitates assistance or interventions for clients between care visits or programs.
  • Promote wellness by providing culturally appropriate health information to providers, patients, and/or community members.
  • Provide individual social and health care support.
  • Refer and link to preventive services through health screenings and healthcare information.
  • Conduct health related screenings as assigned.
  • Assist patients and individuals in setting self-management goals.
  • Provides self-management education based on as assessment of learning needs of patient/care giver.
  • Increase access to primary care through culturally competent outreach and enrollment strategies.
  • Make referrals and coordinate services and facilitate continuity of care by following up.
  • Link patients and individuals to and inform them of available community resources.
  • Articulate and advocate needs of community and individuals to others.
  • Map communities to help locate and support needed services.
  • Educate patients and community members about how to use the health care and social service systems.
  • Educate the health and social service systems about patient and community needs and perspectives.
  • Build community capacity by providing services that address social determinants of health.
  • Mentor other CHWs

  • This is a non-state, time-limited position with The Mississippi State Department of Health.
  • What you’ll need to be Successful Individual completes CHW Certification Training Program.

Individual completes training for and provides disease self-management education to reduce risk factors for chronic disease and/or improve management of chronic conditions. Individual is knowledgeable of and link community members to community resources for
