About the Position This is a time limited NS-14 position. This position is with the Office of Special Disability Programs. The Social Services Program Coordinator identifies and coordinates long-term services and supports to assist individuals with the most severe physical disabilities to remain independent in their homes and communities. Employees in this position oversee ongoing community-based services to these individuals and are responsible for the planning and monitoring of accessibility needs and in-home personal care services to ensure the individuals remain as independent as possible in both the home and community-based setting. Services are provided based on need as determined by a functional assessment and are provided through an individualized plan of services and supports. The Social Services Program Coordinator oversees the process of assessments and reassessment of the individual’s level of care and reviews the plan of services and supports that are completed in the individuals home. Responsibilities will include, but are not limited to:
- Monitoring all approved services to meet the individual’s current needs,
- Conducts quarterly home visits and conduct monthly telephone contacts with the individuals being served.
- Ensuring personal care attendants (PCA’s) chosen by the individual meet basic competencies, and review and approve their payroll.
- Must be able to travel within their assigned area.
What you’ll need to be Successful Good oral and written communications skills are required to be proficient in this position. Must be able to prioritize, plan and provide timely services to those individuals being served. Looking for individuals who can multi-task, are very detailed oriented, analytical, and able to solve problems. Preferred Qualifications Typically requires a Bachelor’s Degree and 2-4 years of experience.
Hiring agency preferred qualifications:
Bachelor’s degree in counseling or related field, and two years of experience working with individuals with disabilities.
